About The Position

This role involves conducting diagnostic assessments under the supervision of a licensed professional, determining clinical interventions, facilitating therapy, and providing education to patients and their families. The Psychotherapist Trainee will manage cases, develop transition/discharge plans, and collaborate effectively with interdisciplinary teams. A key aspect of this position is accepting supervision and consultation from a licensed supervisor for all clinical practice functions.

Requirements

  • Master’s Degree in Psychology, Social Work, or closely related field
  • Completion of clinical education and internship as required by graduate program.
  • Be licensed or provisionally licensed by the MN Board of Social Work as LGSW, or be working towards independent licensure as an LMFT, or LPCC.

Responsibilities

  • Conduct diagnostic assessment under the supervised practice of a fully licensed and system credentialed supervisor.
  • Determine clinical interventions, facilitate therapy, and provide education for patients and their families.
  • Facilitate treatment planning and manage each case.
  • Responsible for the development of transition/discharge plan.
  • Communicate effectively and respectfully with others, including interdisciplinary teams and all other contacts.
  • Function as an interdisciplinary team member.
  • Accept supervision and consultation with a licensed supervisor for the functions of this position in their clinical practice.
  • Demonstrate clinical competence in the treatment planning process.
  • Coordinate/facilitate the development of treatment plan by collaborating with patient, family and interdisciplinary team.
  • Develop an individualized plan based on age, social and cultural considerations, clinical diagnosis, level of functioning and other relevant needs.
  • Implement, reassess and adjust treatment plan as necessary.
  • Demonstrate clinical competence to assess individual patient needs.
  • Complete an interdisciplinary assessment tool with each patient.
  • Update interdisciplinary assessments tool each week.
  • Demonstrate clinical competence in disposition planning.
  • Interface with government agencies such as court, ombudsmen, county services.
  • Complete referral process for next level of care.
  • Communicate to other providers in the community.
  • Set up intake appointments.
  • Complete needed paperwork to facilitate this process i.e. applications, letters of recommendation, etc.
  • Demonstrate clinical competency to partner with multi-disciplinary team in the treatment planning process.
  • Assist in developing an individualized plan based on age, social and cultural considerations, clinical diagnosis, level of functioning and other relevant needs.
  • Communicate frequently and effectively with interdisciplinary team to ensure continuity and coordination of care.
  • Ensure documentation is thorough, timely, legible and clinically appropriate and meets regulatory requirements.
  • Demonstrate clinical competence in developing a clinically relevant transition/discharge plan.
  • Collaborate with patient, family, and provide input to interdisciplinary team in developing a discharge plan.
  • Develop discharge plan based on age, social and cultural considerations, clinical diagnosis and level of functioning and other relevant needs.
  • Document thoroughly, timely, legible and clinically appropriate.
  • Demonstrate clinical competence in conducting group therapy, family therapy and psycho-education groups.
  • Provide individual, couples and family therapy.
  • Advance evidence based practice by utilizing standards, guidelines and pathways for care delivery.
  • Incorporate data and information into practice to continually improve care and practice so that patient outcomes are enhanced.
  • Promote a culture of evidence based practice by participation in learning opportunities about evidence based practice, utilizing evidence based policies in clinical practice, membership in hospital or unit based practice council/research council, evidence based practice projects, journal club, literature review, development of standard/pathway/guideline for patient care, presentations, and research studies.
  • Provide care or service adjusting approaches to reflect developmental level and cultural differences of population served.
  • Partner with patient caregiver in care/decision making.
  • Communicate in a respectful manner.
  • Ensure a safe, secure environment.
  • Individualize plan of care to meet patient needs.
  • Modify clinical interventions based on population served.
  • Provide patient education based on an assessment of learning needs of patient/caregiver.
  • Fulfill all organizational requirements.
  • Complete all required learning relevant to the role.
  • Comply with all relevant laws, regulations and policies.
  • Participate with supervision- attend individual and/or group supervision as agreed upon.
  • Perform other duties as assigned.
