A Psychosocial Rehabilitation Specialist serves in a paraprofessional capacity as part of the crisis intervention unit. The position assists clients with paperwork related to pending admissions, discharges and post-discharge phases, and transportation of patients/clients to and from inpatient psychiatric and crisis stabilization placements. It performs a backup role to the crisis worker with support and informational crisis phone contacts, and assists with the coordination of the care of clients who are placed in crisis stabilization sites. The role also provides in-person medication deliveries, welfare checks, and phone prompts to clients. This position is responsible for assisting clients to develop and implement their personal wellness goals. For adults, the role involves providing rehabilitation support, recovery-oriented activities, interventions, and skill development necessary for individuals to improve self-management and reduce relapse to substance use, and to mitigate the negative effects of psychiatric or emotional symptoms that interfere with daily living skills critical to remaining in home, school, work, and the community. For children, the role includes consulting with collaterals (with client permission) to improve client outcomes, providing psychoeducation to restore community tenure and avoid restrictive placements, enhancing personal relationships, establishing support networks, increasing community awareness, and developing coping strategies for effective functioning in all environments. Both roles involve managing a caseload of clients to establish and implement individual health and wellness plans and assisting in establishing a wellness support system outside of the PRS.
