About The Position

Vail Health is seeking a Psychosocial Oncology Counselor/Social Worker to provide comprehensive psychosocial oncology services to patients across the Shaw Cancer Center (Dillon/Edwards campuses) and the Shaw at Home program. This role is integral to the palliative care team, supporting patients throughout their continuum of care, and also contributes to the Survivorship Program by developing initiatives to enhance survivors' quality of life and reduce recurrence risk. This is a clinical social work position, dedicating 100% of FTE time to direct patient care and clinical functions.

Requirements

  • Two years of previous experience in psychosocial oncology required.
  • State of Colorado Licensed Social Clinical Social Worker (LCSW) or State of Colorado Licensed Professional Counselor (LPC).
  • Master’s degree in Social Work from an institution accredited by the Council of Social Work Education or Master’s degree in Counseling from an institution accredited by CACREP.

  • Oncology Social Worker (OSW-C) preferred.

Responsibilities

  • Utilizes evidence-based practice to function as a practitioner within the field of psychosocial oncology and palliative care.
  • Provides comprehensive psychosocial assessments, palliative care consultations, and develops social work interventions and treatment plans for patients of the Shaw Cancer Center and the Shaw at Home program.
  • Provides support and counseling (1:1, couples, family, and group), assists patient and family with coping skills related to cancer diagnosis and treatment, recovery, palliation, end of life, and bereavement.
  • Responsible for contributing to the development of the Survivorship program through participation in the development, planning, and implementation of survivorship services within the community to promote wellness and cancer risk-reduction initiatives and activities.
  • Responsible for care coordination with community referral partners.
  • Responsible for addressing financial toxicity as it relates to cancer care, collaborating with a financial navigator and directly with patients through education and facilitation of Medicaid buy-in program application completion, and serving as a referral source for community non-profits.
  • Serves as liaison to staff to help problem-solve departmental issues/concerns to promote communication and cooperation.
  • Serves as a resource to colleagues of the Shaw Cancer Center for support and counseling to assist staff dealing with patient situations that can result in vicarious trauma or with the imbedded stressors of working within a healthcare environment.
  • Represents the Shaw Cancer Center and Vail Health by attending and speaking at community events, in marketing campaigns, professional meetings, and in various other media outlets.

  • Perform other duties as assigned.
