About The Position

Telos Health Systems is seeking an experienced Psychiatry Advanced Practice Provider (APP) to provide psychiatric and behavioral health services to residents in Skilled Nursing Facilities (SNFs) and Assisted Living Facilities (ALFs) throughout the Asheville area. We are seeking a Physician Assistant (PA-C) with a Certificate of Added Qualifications (CAQ) in Psychiatry (preferred) or significant experience in geriatric behavioral health. This provider will work collaboratively with facility staff, primary care providers, and interdisciplinary teams to improve residents' mental health, quality of life, and overall clinical outcomes. This position is ideal for a compassionate, clinically confident provider who values autonomy, flexibility, and meaningful relationships with patients in long-term care. Requirements

  • Active, unrestricted North Carolina Physician Assistant (PA-C) license
  • Current NCCPA certification
  • Master's degree from an accredited Physician Assistant program
  • Current DEA registration
  • Strong clinical assessment and communication skills
  • Ability to practice independently while collaborating within an interdisciplinary team

Nice To Haves

  • Certificate of Added Qualifications (CAQ) in Psychiatry
  • Experience in geriatric psychiatry
  • Skilled Nursing Facility (SNF), Assisted Living (ALF), or long-term care experience
  • Behavioral health or psychiatric practice experience
  • Knowledge of dementia-related behavioral disturbances and psychopharmacology

Responsibilities

  • Provide comprehensive psychiatric evaluations for SNF and ALF residents
  • Diagnose and manage psychiatric and behavioral health conditions
  • Develop individualized treatment plans
  • Prescribe and manage psychotropic medications
  • Monitor medication effectiveness and side effects
  • Complete routine psychiatric follow-up visits
  • Evaluate residents experiencing acute changes in mood, behavior, cognition, or mental status
  • Participate in behavioral health care planning and interdisciplinary team meetings
  • Collaborate with primary care providers, medical directors, nursing staff, therapists, and social workers
  • Support facilities in reducing unnecessary psychiatric hospitalizations
  • Complete timely documentation and accurate CPT coding
  • Participate in advance care planning discussions as appropriate
  • Coordinate referrals to specialty behavioral health services when indicated
