About The Position

The Suffolk County Department of Health Services is seeking to fill the position of Psychiatric Social Worker within the Children’s Single Point of Access (SPOA) unit in Hauppauge. This role involves reviewing applications for high-intensity mental health services for children and youth diagnosed with serious mental illness, conducting strength-based assessments, determining levels of need, recommending services, and linking clients to appropriate resources. The position requires direct client contact with youth and their families, preparing case records, reviewing financial aspects of service programs, and attending meetings with mental health professionals and multi-disciplinary teams. Additionally, the role involves providing outreach, crisis intervention, and support, as well as assisting families and providers in navigating child-serving systems.

Requirements

Graduation from a college with federally-authorized accreditation or registration by NY State with a Master's Degree in Social Work
Certification as Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) by the New York State Department of Education.
Certification as Licensed Master Social Worker (LMSW) or a Licensed Clinical Social Worker (LCSW) by the New York State Department of Education must be maintained throughout employment in this title.
Possess a valid license to operate a motor vehicle in New York State or otherwise demonstrate their capacity to meet the transportation needs of the job.
Thorough knowledge of the principles, methods and practices of psychiatric social work
Thorough knowledge of the functioning of mental hygiene out-patient services
Thorough knowledge of the social and psychological aspects of mental disease, mental retardation, developmental disabilities, chemical dependency and of modern treatment methods
Good knowledge of community resources and how to use them effectively
Ability to develop and maintain cooperative relationships with patients, colleagues, patients' families, physicians and social agencies
Ability to prepare comprehensive case work and treatment records
Ability to express oneself clearly and concisely, both orally and in writing
Ability to analyze information and reach sound conclusions

Responsibilities

Reviews applications received by the Suffolk County Children’s Single Point of Access.
Direct client contact with youth and their families, including referral process, strength-based assessment, level of need determination, service recommendation and linkage to CSPOA and/or alternative community resources.
Prepares case records with corresponding documentation into relevant databases/EHR, accurately and efficiently.
Reviews financial aspects of service programs with youth/family/provider, including service dollar requests.
Attends meetings with mental health and other professionals to discuss diagnoses and service recommendations for youth with active engagement.
Attends multi-disciplinary cross system teams/child service system meetings with active engagement.
Assists with general mental hygiene services work, including C&Y program support, administrative support, committee meetings, and DD related, etc.
Provides outreach services and assists families/providers navigate multiple child-serving systems.
Provides crisis intervention and support.
Performs other duties as required.
