The Washington State Penitentiary is seeking a Psychiatric Social Worker 3 (PSW3) to provide medical, behavioral health, and substance use disorder transition planning services. This role supports the Department's mission to improve public safety by assessing the transition needs of incarcerated individuals preparing for release, coordinating and advocating for necessary services, providing resources, and developing Continuity of Care Plans. These plans assist patients with significant physical and/or mental disorders in their transition back to the community. The PSW services are guided by the DOC agency core values of integrity, trust, respect, and inclusivity. Continuity of Care aims to help patients succeed, reduce recidivism, and contribute to prison and community safety, aligning with the DOC's commitment to Equity, Diversity, Inclusion, and Respect. This position, in collaboration with the facility Health Services reentry team, focuses on the Department's Strategic Goal of Continuity of Care Planning for Health Services patients.
