About The Position

Responsible for providing primary nursing support to medication services/clinic clients. The essential functions include, but are not limited to, maintaining standards of agency and credentialing policies and procedures, participating in ongoing QI activities, and participating as a member of the UM Per Panel. As a member of the Medical Service Team, the individual will actively participate in Medical Service meetings and program-related meetings. This role involves completing medical records and other documentation in a timely and accurate manner, coordinating with psychiatrists and pharmacies for medication management, and consistently meeting billable expectations. The position also includes coordinating and facilitating telehealth appointments between clients and providers, completing prior authorizations for medications, and maintaining a safe, hygienic service provision environment. Administering injectable medication and providing appropriate client education with respect to medication is a key responsibility. The role requires taking calls from hospitals, doctors, nurses, and other mental health professionals to coordinate client care, triaging walk-in clients, and providing nursing assessment, supportive therapy, crisis intervention, and exploration of therapeutic options. Facilitating and participating in offsite medication clinics in the service area, referring clients to therapists, Day Programs, Partial Hospital, Hospital, or other providers, and working in a cooperative and problem-solving manner with other agency departments and community agencies are also essential. The individual will represent the agency in a cooperative, positive manner, identifying and reporting any concerns regarding the improvement of the service delivery system. Providing back-up for crisis to non-medical QMHP and maintaining knowledge of agency technology programs and/or systems needed to do the job and using them effectively are also part of the role. Assisting all Med Team members as needed and performing other duties and/or responsibilities as assigned by the supervisor are expected.

Requirements

  • Registered Nurse preferred, LPN required
  • CPR/First Aid certification required
  • Active Driver’s License with minimum state required insurance
  • 2+ years of similar position-related experience preferred.
  • Knowledge gained through prior employment and/or education related to targeted health, along with chronic disease processes, especially related to obesity, diabetes, hypertension, COPD/asthma, and heart disease.
  • Excellent phone and verbal communication skills.
  • Proficient computer skills to include Microsoft Word and Outlook.
  • Experience with electronic medical records.
  • Proficiency with point-of-care metabolic screening testing and vital sign collection used for metabolic syndrome screenings.
  • Excellent patient/consumer education skills.
  • Excellent organization and prioritization skills.

  • Experience in behavioral health and/or substance use fields preferred.
