About The Position

We are seeking a dedicated and compassionate Psychiatric Rehabilitation Service Coordinator (PRSC) to join our team at Oak Park Oasis. As a PRSC, you will play a crucial role in ensuring the seamless transition of patients from hospital to home, providing high-quality care that sets us apart as a Joint Commission Approved facility. In this full-time position, you will work closely with our multidisciplinary care team to develop individualized plans of care for patients with complex psychiatric and rehabilitation needs. Your responsibilities will include conducting comprehensive assessments of patients' condition prior to discharge from the hospital, creating tailored plans of care, monitoring progress, and communicating with patients' physicians and families. You will also coordinate additional outside services to ease the transition back home and ensure continuity of care, and collaborate with our medical director and other healthcare professionals to achieve outstanding patient outcomes.

Requirements

• Bachelor's degree
• 1 year of experience

Responsibilities

• Conduct comprehensive assessments of patients' condition prior to discharge from the hospital
• Develop individualized plans of care in collaboration with the multidisciplinary care team
• Implement and monitor patients' care plans, making adjustments as needed
• Communicate with patients' physicians and families to ensure seamless care transitions
• Coordinate additional outside services to support patients' independence and well-being
• Participate in multidisciplinary care team meetings to share knowledge and best practices
