About The Position

This role involves participating in the development of individualized rehabilitation plans, ensuring timely client appointments, and adhering to the Caregivers Policy on transportation. The position also includes participating in data collection for the quality assurance plan, documenting client hospitalizations, and completing all documentation according to OMHSAS and Program guidelines. A key aspect is maintaining a caseload and a flexible schedule, which may include evenings or weekends for orientation to community programs. The role requires planning, organizing, and implementing individual goal-directed activities, developing a client-driven curriculum, and managing communication with team members regarding client caseloads. Additionally, the position involves scheduling and attending team meetings, completing billing authorizations, documenting client progress after each session, and meeting with supervision bi-weekly. Other duties as assigned.

Requirements

  • High School Diploma, GED, or Higher Level of Education
  • Two (2) years’ experience in human services, must include (1) year of experience in Mental Health direct service.
  • Strong leadership ability, good organizational skills, independent and critical thinking skills, sound judgment, and knowledge of legal aspects and liability of nursing practice.
  • Strong ability to communicate complex and/or controversial topics and concepts to a wide and diverse audience.
  • Current Basic Life Support (BLS) approved curriculum of the American Heart Association (CPR and AED) program.
  • Valid Driver’s License (if out of state hire, the record report from applicable state’s driver license department is required).
  • Act 33 Clearance with renewal.
  • Act 34 Clearance with renewal.
  • Act 73 FBI Clearance with renewal.
  • Child mandated court Reporting Training within 30 days of hire or transfer.

Nice To Haves

  • Experience in Healthcare setting.
  • Must obtain Certified Psychiatric Rehabilitation Practitioner (CPRP) within two years of hire or transfer.

Responsibilities

  • Participates in the development of individualized rehabilitation plans.
  • Ensure that all client appointments are set in a timely fashion.
  • Follows Caregivers Policy on transportation.
  • Participates in the collection of data for the comprehensive quality assurance plan to evaluate the program.
  • Document all in-patient hospitalizations to clients participating in the Psych Rehab program.
  • Completes all documentation according to OMHSAS and Program guidelines.
  • Maintain a caseload.
  • Maintain a flexible schedule.
  • May have to work evenings or weekend for orientation to community programs.
  • Plan, organize, and implement individual goal directed activities.
  • Develops a client driven curriculum consistent with client needs and choices.
  • Responsible for calling and returning phone calls to various team members of your client caseload.
  • Responsible for scheduling and attending team meetings and STAR meetings as needed.
  • Complete and submit billing authorization for clients.
  • Document after each session a note that describes how it helps each client work towards his or her goal.
  • Schedule and meet with supervision every two weeks to update on caseload and any other issues.
  • Other duties as assigned.
