About The Position

Elm & Oak Health is seeking a part-time Psychiatric Provider to join their Safe Options Support (SOS) teams. This role involves delivering psychiatric care to unsheltered individuals experiencing serious mental illness and co-occurring substance use disorders. The provider will conduct field visits and telehealth to offer evaluations, medication management, and harm-reduction-oriented care, while also providing clinical leadership to a multidisciplinary team. The position is ideal for a psychiatric NP, MD, or DO looking for flexible, impactful work. Requirements

Active, unrestricted NYS license as a Nurse Practitioner (NP), Medical Doctor (MD), or Doctor of Osteopathic Medicine (DO)
Board certification or board eligibility in Psychiatry (or Psychiatric-Mental Health Nurse Practitioner certification)
Valid DEA registration with ability to prescribe controlled substances in NYS
Experience providing psychiatric care to individuals with serious mental illness, substance use disorders, and/or co-occurring conditions
Familiarity with PSYCKES and electronic medical record (EMR) documentation
Ability to work in community-based, field, and telehealth settings
Punctuality and reliable attendance
Knowledge of homeless resources, shelter systems and transportation systems.
Knowledge of counseling principles and methods for mental illness and substance use disorders.
Knowledge of treatment, rehabilitation, and community support programs as they relate to recipient/residents, families, and staff.
Demonstrates ability to respect individual/family diversity and maintain confidentiality.
Demonstrates ability to work as a team member.
Knowledge of and ability to work collaboratively with providers and county/community health and human services.
Ability to demonstrate excellent communication skills both oral and written as well as strong interpersonal skills.
Proven ability to work independently and to manage time appropriately.
Strong organizational skills.
Computer literate.
Must be able to pass computer documentation competency testing for all software platforms used within the program.
Candidates will need a NYS driver’s license and to own or have access to reliable transportation that enables them to fulfill travel requirements of the job including but not limited to, daily visits to members’ homes.

Nice To Haves

Experience with street medicine, assertive community treatment (ACT), or other outreach-based psychiatric care models
Experience administering long-acting injectable antipsychotics and/or Medication Assisted Treatment (MAT), including buprenorphine and extended-release naltrexone
Familiarity with clozapine therapy protocols and monitoring requirements
Experience working with individuals experiencing homelessness, housing instability, or involvement in the criminal justice system
Training in trauma-informed care, motivational interviewing, and/or de-escalation techniques
Bilingual or multilingual abilities a plus

Responsibilities

Performs diagnostic evaluations, provides psychotherapy, and implements psychiatric medication regimens for SOS participants.
Utilizes flexible modalities including field visits, engagement encounters, and telepsychiatry to improve access.
Implements long-acting injectable medications, Medication Assisted Therapy (MAT), clozapine therapy, and medication delivery support as clinically appropriate.
Applies evidence-based, harm-reduction-oriented prescribing practices.
Consults, advises, and coordinates with non-SOS psychiatric providers.
Collaborates with the SOS team to communicate with hospitals and crisis services.
Coordinates care transitions between practitioners.
Reviews PSYCKES and other clinical data to inform treatment decisions.
Provides medical leadership for the SOS team.
Collaborates with the team to develop evidence-based clinical treatment plans.
Identifies staff clinical educational needs.
Delivers clinical trainings and provides input on case management support.
Reviews quality of team-provided care and collaborates with the Team Lead to continuously improve care quality.
Co-leads SOS Clinical Team Meetings and SOS Clinical Leadership Meetings.
Maintains the psychiatric treatment schedule in collaboration with the Clinical Liaison.
Participates in sign-in and sign-out with the Clinical Liaison at the beginning and end of each shift.
Documents all treatment encounters, medication changes, and recommendations.
Adheres to SOS guidelines and key performance indicators as specified by OMH street psychiatry guidance.
Demonstrates cultural awareness, cultural knowledge, and cultural skills.
Attends and participates in team meetings and organizational/departmental trainings.
Performs other duties as assigned.
