About The Position
A 64-bed adult psychiatric facility, Lasting Hope Recovery Center is a place of healing. All of the rooms are private. At Lasting Hope Recovery Center, patients find comfort and a team of caring professionals working for them. Often, mental illness and substance abuse occur at the same time. CHI Health Lasting Hope Recovery Center offers an entire range of treatment and prevention services to meet the needs of patients experiencing both challenges. Treatment is built on personal strengths and support systems that treat individuals experiencing these illnesses.
Requirements
- Graduate of an RN nursing program
- Registered Nurse: NE
- Basic Life Support - CPR

Responsibilities
- Deliver compassionate, high-quality care that impacts patients' well-being and recovery.
- Provide individualized, comprehensive care.
- Make critical assessments.
- Perform skilled procedures.
- Implement patient care plans.
- Collaborate within an interdisciplinary team.
- Follow the nursing process: assess, plan, implement, and evaluate.
- Collect and analyze healthcare consumer data including: psychological, physiological, sociocultural, spiritual, economic and lifestyle factors.
- Set measurable and achievable short and long term goals according to standards of care based on the assessment by the nurse in collaboration with the healthcare consumer and multidisciplinary team.
- Implement and/or modify the plan of care using assessment data, clinical reasoning based upon standards of care and evidence based practice.
- Provide care coordination, education and discharge planning.
- Accountable for the care directly, indirectly or through delegation, following professional and legal standards for delegation.
- Use a systematic ongoing process to evaluate progress towards identified healthcare consumer goals.
- Revise plan of care when needed involving the healthcare consumer and the multidisciplinary team.
- Document all phases of the nursing process in the healthcare consumer’s record.
