Learn more at: www.envisionunlimited.org ACT Psychiatric Nurse (RN) Location: Chicago, IL 60640 (On-site) Job Type: Full-Time Salary: $85,000 per year Make a Difference Every Day We are seeking a compassionate Registered Nurse (RN) to join our Assertive Community Treatment (ACT) team. In this role, you'll provide community-based psychiatric nursing services to adults living with serious mental illness while collaborating with an interdisciplinary team dedicated to promoting recovery and independence.

Requirements

  • Current Illinois Registered Nurse (RN) license.
  • Minimum of 2 years' experience working with individuals with serious mental illness.
  • Valid Illinois driver's license and ability to travel within the community.
  • Ability to pass a background check and complete required annual training.

Responsibilities

  • Coordinate medical care and annual health evaluations for ACT clients.
  • Administer injectable psychotropic medications under psychiatric direction.
  • Monitor lab results, vital signs, medications, and medical appointments.
  • Coordinate with physicians, pharmacies, hospitals, and community providers.
  • Conduct home visits as needed and respond to medical emergencies.
  • Participate in treatment planning, clinical meetings, and maintain timely Medicaid Rule 132/140 documentation.
  • Provide education and support to clients, caregivers, and staff.
