About The Position

We are actively seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) to join a growing behavioral health program supporting residents throughout skilled nursing facilities, rehabilitation centers, long-term care communities, and post-acute settings across Erie and Niagara County. This part-time opportunity is designed for clinicians who value flexibility, independence, and meaningful patient impact. Providers will work with an aging population experiencing complex psychiatric, cognitive, medical, and behavioral health conditions while becoming an integral part of facility care teams focused on improving stability, quality of life, and continuity of treatment. Requirements

  • Board Certified Psychiatric Mental Health Nurse Practitioner (PMHNP)
  • Active and unrestricted New York State Nurse Practitioner license
  • DEA registration preferred or ability to obtain
  • Comfortable practicing independently within long-term care environments
  • Prior exposure to geriatric psychiatry, behavioral health, skilled nursing, rehabilitation, post-acute, or nursing home populations preferred
  • Experience managing dementia-related symptoms, chronic psychiatric illness, and psychotropic medication regimens strongly preferred
  • Strong interpersonal, organizational, and communication skills
  • Ability to work collaboratively within interdisciplinary healthcare settings

Responsibilities

  • Conduct psychiatric assessments and behavioral health evaluations within long-term care and skilled nursing environments
  • Provide ongoing psychopharmacology management and psychiatric follow-up visits
  • Evaluate and manage conditions including depression, anxiety, psychosis, mood disorders, dementia-related behaviors, chronic psychiatric illness, and behavioral disturbances
  • Adjust and monitor psychiatric medications based on resident presentation and treatment response
  • Develop individualized psychiatric care plans in collaboration with interdisciplinary teams
  • Work closely with nursing staff, therapists, physicians, social workers, and facility administrators regarding resident care needs
  • Support behavioral management strategies and continuity of psychiatric treatment within facility settings
  • Maintain timely, organized, and compliant clinical documentation
  • Communicate effectively with residents, family members, caregivers, and facility staff
  • Participate in maintaining quality behavioral health services across assigned facilities
