A growing behavioral health organization is seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide on-site psychiatric services within nursing homes, skilled nursing facilities, rehabilitation centers, and long-term care communities throughout Oneida County, New York and surrounding communities. Requirements

  • Board Certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC)
  • Active and unrestricted New York State Nurse Practitioner license
  • DEA registration or ability to obtain
  • Strong background in psychiatric assessment, diagnosis, psychopharmacology, and treatment planning
  • Ability to independently manage a psychiatric caseload while collaborating effectively with interdisciplinary teams
  • Excellent communication, relationship-building, and patient education skills
  • Strong organizational skills and commitment to quality clinical care

Nice To Haves

  • Experience working with adult, geriatric, skilled nursing, rehabilitation, long-term care, behavioral health, or post-acute populations preferred
  • Experience managing dementia-related symptoms, chronic psychiatric illness, mood disorders, anxiety disorders, psychosis, and behavioral disturbances strongly preferred

Responsibilities

  • Conduct initial psychiatric consultations, comprehensive assessments, and ongoing behavioral health evaluations for residents in nursing home and long-term care environments.
  • Diagnose and manage psychiatric conditions commonly affecting adult and geriatric populations, including depression, anxiety disorders, psychotic disorders, mood disorders, adjustment disorders, sleep disturbances, chronic mental illness, and behavioral complications associated with cognitive decline.
  • Provide medication management services, including psychotropic medication initiation, monitoring, optimization, and adjustment.
  • Evaluate residents experiencing dementia-related behaviors, agitation, confusion, aggression, emotional distress, and other neuropsychiatric symptoms.
  • Develop individualized treatment strategies that address psychiatric, cognitive, medical, functional, and psychosocial needs.
  • Collaborate with nursing staff, rehabilitation teams, attending physicians, medical directors, social workers, and facility leadership to support integrated patient care.
  • Participate in interdisciplinary care planning meetings and behavioral management discussions.
  • Educate facility staff, residents, and family members regarding diagnoses, medication management, treatment goals, and behavioral health interventions.
  • Support efforts to reduce avoidable psychiatric crises, emergency department utilization, and unnecessary hospitalizations.
  • Ensure compliance with documentation standards, regulatory requirements, and organizational quality initiatives.
  • Maintain a patient-centered approach that emphasizes dignity, compassion, safety, and continuity of care.
