Requirements

  • Active, unrestricted PMHNP-BC or PA license in the state of practice.
  • Minimum 1-2 years of clinical experience (geriatrics, primary care, home-based care, or value-based care preferred).
  • A highly professional, dependable, and trustworthy approach to patient care.
  • Strong clinical judgment and the ability to work independently with confidence.
  • Excellent relationship-building and communication skills.
  • An entrepreneurial mindset with a passion for growing provider-community relationships.
  • Adaptability, accountability, and a commitment to delivering exceptional patient experiences.
  • Valid driver's license and reliable transportation.

  • Managing your own patient panel.
