About The Position

The Multnomah County Health Department is seeking a visionary and dynamic Psychiatric Mental Health Nurse Practitioner (PMHNP) to join their Integrated Clinical Services team. This role is crucial in advancing health equity, protecting vulnerable populations, and promoting health and wellness for everyone. The PMHNP will provide professional, comprehensive, and specialized psychiatric nursing services with an emphasis on disease prevention and health promotion. This position involves establishing therapeutic relationships with a diverse patient population, diagnosing and treating acute and chronic mental health conditions across the lifespan, ordering and interpreting diagnostic tests, prescribing and monitoring medications, and providing patient education. The role also includes indirect clinical care, such as consulting with other providers, coordinating with interdisciplinary teams, and participating in meetings and continuing education. Requirements

  • Masters degree or equivalent in nursing from an accredited college or university
  • Possession of a valid Psychiatric Nurse Practitioner Certificate
  • License to practice as a Registered Nurse in the State of Oregon
  • CPR/BLS certification
  • DEA certification
  • Demonstrated knowledge of the principles, practices and procedures of psychiatric medicine
  • Demonstrated knowledge of methods and techniques of diagnosing and treating a variety of diseases
  • Ability to effectively screen and gather required information in a professional, culturally competent and customer service oriented manner
  • Ability to motivate clients through discussion and persuasion
  • Ability to communicate clearly and concisely, both orally and in writing
  • Ability to establish and maintain cooperative working relationships with those contacted in the course of work
  • Ability to be flexible and manage multiple priorities
  • Ability to maintain composure and work objectively with clients who may be hostile or verbally abusive

Responsibilities

  • Provides professional, comprehensive specialized psychiatric nursing services with an emphasis on prevention of disease and promotion of health.
  • Establishes a therapeutic relationship with a diversity of patients.
  • Diagnoses and treats acute and chronic mental health conditions and diseases according to scope of practice across the lifespan.
  • Conducts a medical history and a mental status exam.
  • Prescribes mental health treatment, including ordering and interpreting diagnostic tests.
  • Devises a course of treatment, discusses alternative treatment, risks and benefits of treatment.
  • Prescribes and monitors medications.
  • Provides appropriate counseling of patient and reassessment of the patient at appropriate intervals.
  • Provides patient education on disease treatment options, nutrition, exercise, and health maintenance behavior.
  • Maintains high-quality notes and recommendations for treatment and next steps in the care plan as part of a team-based approach.
  • Seeks clinical consultation from other clinicians as needed as part of practice management.
  • Provides consultation to other providers, nurses and support staff.
  • Prioritizes and directs activities of the care team.
  • Coordinates with primary care providers, behavioral health, pharmacy, and dental providers to effectively integrate care.
  • Provides coverage for clinical colleagues in their absence.
  • Completes patient documentation and manages electronic in-basket according to established policies and procedures.
  • Follows up on utilization issues (ED, hospital admissions) including specialty referrals and consultations.
  • Participates in Team, Professional, and Health Center-wide meetings.
  • Participates in continuing education to maintain licensure, and current, evidence-based practice.
  • Creates a work culture of respect, trust and understanding for the highly diverse populations the Health Department serves in the office and in the community.
