We are seeking an experienced and collaborative Psychiatric Mental Health Nurse Practitioner (PMHNP) to support our Charlottesville team on a part-time basis. This role provides high-quality, person-centered psychiatric assessment, diagnosis and treatment within post-acute, long-term care and assisted living settings. The successful candidate will evaluate mental health and behavioral concerns, develop and manage individualized treatment plans, monitor treatment effectiveness, support medication management, coordinate care with interdisciplinary teams and promote safe, effective continuity of care.

Requirements

• Master’s degree or Doctor of Nursing Practice (DNP) from an accredited Psychiatric Mental Health Nurse Practitioner programmed
• Current, unrestricted Virginia Psychiatric Mental Health Nurse Practitioner license and eligibility to practice in Virginia
• Current national board certification as a Psychiatric-Mental Health Nurse Practitioner through the appropriate certifying organization
• Current prescriptive authority and Drug Enforcement Administration (DEA) registration, as required
• Experience in psychiatric mental health, post-acute, long-term care, assisted living, geriatric, outpatient, primary care, hospital or related clinical settings preferred
• Experience caring for older adults and patients with complex psychiatric, medical and behavioral health needs preferred
• Strong psychiatric assessment, diagnostic reasoning, risk assessment and treatment planning skills
• Knowledge of psychopharmacology, chronic mental health conditions, dementia-related behavioral symptoms, substance use concerns and care coordination
• Proficiency with Electronic Medical Record systems and timely, accurate documentation practices
• Excellent communication, collaboration, organization and problem-solving abilities
• Compassionate, culturally sensitive, trauma-informed and person-centered approach to patient care
• Ability to work independently while collaborating effectively with physicians and multidisciplinary care teams
• Availability to provide reliable part-time PMHNP coverage based on patient and facility needs

Responsibilities

• Provide comprehensive psychiatric mental health assessment, diagnosis and treatment to patients in post-acute, long-term care and assisted living settings
• Conduct initial, routine and follow-up psychiatric evaluations, including review of mental health history, symptoms, medications, diagnoses and current concerns
• Assess and manage a range of psychiatric, psychological and behavioral health conditions within the scope of practice
• Evaluate mood, affect, thought processes, cognition, behavior, functional status, safety risks and changes in clinical presentation
• Develop, implement and review individualized psychiatric treatment plans in collaboration with patients, families, facility staff and interdisciplinary care teams
• Prescribe, review and manage psychiatric medications in accordance with applicable regulations, clinical guidelines and patient needs
• Monitor treatment response, therapeutic outcomes, adverse effects, drug interactions, polypharmacy and clinical risk
• Assess and manage changes in mental health status, acute psychiatric needs, behavioral concerns and urgent patient issues, escalating care appropriately when required
• Evaluate patients for suicidal ideation, self-harm, aggression, psychosis, delirium and other safety concerns, initiating appropriate interventions and referrals
• Order, review and follow up on relevant laboratory tests, diagnostic investigations and other clinical information related to psychiatric treatment
• Collaborate with physicians, nurse practitioners, nurses, psychologists, therapists, specialists, hospitals and community mental health resources
• Participate in care planning meetings, interdisciplinary reviews, family consultations and facility clinical meetings as required
• Support appropriate transitions of care, hospital follow-up and coordination of psychiatric and behavioral health services across the continuum
• Provide education and support to patients, families and facility staff regarding diagnoses, medications, treatment plans, coping strategies, symptom management and prevention of clinical deterioration
• Promote evidence-informed psychiatric practice, patient safety, trauma-informed care, infection prevention, risk management and quality improvement
• Maintain accurate, timely and clinically appropriate documentation in the Electronic Medical Record (EMR)
• Communicate effectively with the supervising or collaborating physician and other members of the care team, as applicable
