About The Position

Avail Health is establishing a behavioral health and dementia care program focused on Medicare patients with serious mental illness, complex psychiatric conditions, substance use disorders, and dementia-related needs. The Psychiatric Mental Health Nurse Practitioner (PMHNP) will be the central clinical psychiatric figure in this program, responsible for comprehensive psychiatric evaluations, medication management, and dementia care delivered via telehealth. This role operates within a coordinated interdisciplinary team and involves serving as the GUIDE Provider of Record for Avail Health's participation in the CMS GUIDE model, offering clinical oversight and expertise in dementia care. The PMHNP will collaborate with an LCSW, Dementia Care Navigator, and Care Coordinator, utilizing an ambient AI scribe and technology-enhanced workflows to minimize administrative tasks and ensure high-quality virtual care. This is a key foundational clinical position in a newly launched program.

Requirements

  • MSN or DNP with a specialization in psychiatric mental health nursing from an accredited program.
  • Active, unrestricted APRN licensure in the State of Maryland in good standing.
  • Board certification as a PMHNP-BC through ANCC or equivalent certifying body.
  • Active Maryland DEA registration including Schedule III–V controlled substance prescribing authority.
  • Minimum 5 years of post-certification clinical experience in psychiatric advanced practice with demonstrated expertise in psychiatric evaluation, medication management, and geriatric mental health.
  • Extensive clinical experience in dementia evaluation and management, including pharmacological management of BPSD.
  • Clinical experience in SUD diagnosis and treatment, including initiation and monitoring of medication-assisted treatment.
  • Experience ordering and coordinating home health services including behavioral health nursing, PT, and skilled nursing.
  • Proficiency with EHRs and telehealth platforms; comfort with technology-enabled clinical workflows.
  • Reliable high-speed internet and a dedicated, HIPAA-compliant home workspace.

Nice To Haves

  • Fluency or conversational proficiency in Spanish.
  • Fellowship or advanced training in geriatric psychiatry.
  • Experience as a provider of record within a CMS Innovation Center model or similar value-based care program.
  • Familiarity with the CMS GUIDE model or other dementia-focused care navigation programs.
  • Familiarity with Medicare Advantage, TCM, or value-based care models.
  • Experience with ambient AI scribe tools or other technology-enabled clinical platforms.
  • Experience within startup healthcare organizations, new program launches, or rapidly scaling care environments.

Responsibilities

  • Conduct comprehensive psychiatric diagnostic evaluations, including history-taking, mental status examination, and DSM-5 diagnostic formulation for mood, anxiety, psychotic, trauma-related, and late-life psychiatric presentations.
  • Prescribe, titrate, and monitor psychotropic medications, considering geriatric pharmacology, polypharmacy risks, drug-drug interactions, and comorbid medical conditions.
  • Assess, diagnose, and provide pharmacological treatment for substance use disorders, including initiating, titrating, and monitoring medication-assisted treatment with buprenorphine/naloxone.
  • Evaluate and manage patients across the dementia spectrum, including pharmacological management of behavioral and psychological symptoms of dementia (BPSD).
  • Serve as GUIDE Provider of Record, providing clinical oversight for care plans, collaborating with the Dementia Care Navigator and LCSW, and contributing to dementia care protocol development in alignment with CMS requirements.
  • Support patients transitioning from inpatient psychiatric or medical settings, including medication reconciliation, psychiatric stabilization, and care continuity.
  • Conduct clinical risk assessments, develop individualized safety plans, and coordinate escalation with care team members and emergency services when indicated.
  • Participate in interdisciplinary huddles and case reviews with the LCSW, Dementia Care Navigator, Care Coordinator, and clinical leadership.
  • Complete all clinical documentation accurately within 48 hours of encounter, meeting applicable regulatory, clinical, and billing standards.
  • Contribute frontline clinical insight to support workflow refinement, protocol development, and scalable program infrastructure.
