The Advanced Practice Nurse/PMHNP will be responsible for conducting patient visits in accordance with regulations, communicating findings to the interdisciplinary team, facilitating access to support models of care, consulting with the team on patient care plans, preparing and maintaining accurate patient records, assisting in the development of clinical practice guidelines, and ensuring collaboration with patients, caregivers, medical directors, and the IDT. The role requires a Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program, a Nurse Practitioner's license in the state of operations, PMHNP certification, and at least 1 year of experience in Family, General Practice, Geriatric care, or a related field. Experience in a nursing home, hospital, assisted living, or acute rehab space is a plus. A commitment to ongoing professional development, a valid driver's license, and reliable transportation are also required, along with a willingness to travel locally.
