About The Position

TVHC is a Federally Qualified Health Center (FQHC) dedicated to promoting community health through accessible, high-quality integrated care. The Psychiatric Mental Health Nurse Practitioner (PMHNP), as part of a multi-disciplinary team led by the Director of Behavioral Health Services and in collaboration with the Chief Medical Officer and Director of Population Health, will work across departments to reduce healthcare access barriers. Providers are expected to adhere to standard medical practices, health center policies, and deliver culturally sensitive care to a diverse, multicultural, and multilingual patient population in an outpatient primary care setting.

Requirements

  • Board certification from a certified Nurse Practitioner or related program which entails training as well as practice in the above-mentioned duties.
  • Current, valid license to practice medicine in the State of California, including DEA license and CPR required.
  • Excellent problem-solving skills required, including creativity, resourcefulness, timeliness, and technical knowledge related to analyzing and resolving medical problems.
  • Sensitivity to needs of culturally and linguistically diverse patient and employee populations.
  • Demonstrated ability to utilize computer technology and to willingness to develop and adapt to the evolving technological requirements of modern medical health center practices required.

Nice To Haves

  • Two years of experience as a Psychiatric Mental Health Nurse Practitioner preferred, but not required.
  • Previous experience providing medical care within a community health setting preferred.
  • Bilingual, especially Spanish, is preferred but not required.

Responsibilities

  • Performs comprehensive mental health/behavioral health assessments, including history and physical examination, diagnostic tests, medication management, and psychosocial/family factor evaluation for diagnosing and managing common acute and chronic psychiatric conditions, and coordinating follow-up care.
  • Provides regular and ongoing medication management for psychiatric conditions, including initiation, titration, and monitoring of psychotropic medications.
  • Manages common acute and chronic mental illnesses and initiates life-saving procedures in emergencies.
  • Works directly with Behavioral Health and Medical departments to support integrated, whole-person care.
  • Consults with the Director of Behavioral Health Services and collaborates with the Chief Medical Officer or designee for clinical guidance, complex case review, and oversight.
  • Promotes preventive mental health care through periodic assessments, screenings, and patient guidance on behavioral health maintenance and healthy habits.
  • Discusses and refers complex or high-priority patient cases.
  • Works as a member of the healthcare team to educate patients and the community on preventive mental health and behavioral health concepts.
  • Directs medical assistants, lab personnel, and other non-professional medical staff as appropriate.
  • May provide in-service training for staff on selected topics within scope of practice.
  • Performs all duties for successful electronic health records maintenance, including completion of medical records and documentation, inbox and results management, coding, and timely chart sign-off.
  • Performs internal quality control audits and maintains standards of care consistent with specialty and organizational requirements.
  • Performs all duties and services in full compliance with the organization’s Service Excellence Standards and adheres to all applicable codes of ethics and the organization’s code of conduct for clinical providers.
  • Demonstrates a commitment to continued professional learning and growth as a practitioner.
  • Attends internal and external meetings, workshops, and conferences, and participates on task forces or committees, as necessary and/or required.
  • Willing to travel to and from service sites as needed.
  • Provides additional assistance within and outside of the clinical arena, as requested and/or as appropriate, to ensure the ongoing success of the organization.
  • Works as a member of the health care team and assists in education of patients and the community in preventive mental health care and behavioral health maintenance concepts.
  • Other duties or responsibilities as assigned by the Director of Behavioral Health Services.
