Psych Provider - Geriatric Care
About The Position
Senior Doc, part of Brigade Health & Affiliates, is seeking a mission-driven Psychiatric Mental Health Nurse Practitioner (PMHNP) to deliver high-quality geriatric psychiatry and behavioral health services. This role involves providing care in various settings including homes, assisted living facilities, memory care units, and skilled nursing facilities. The practitioner will build and manage their own patient panel, collaborating with primary care, palliative care, and neurology teams. The position emphasizes an integrated, continuum-based behavioral health model, focusing on conditions such as dementia-related behaviors, depression, anxiety, psychosis, mood disorders, and complex psychiatric comorbidities. It offers significant autonomy, supported by strong operational and clinical resources, and provides a clear path for clinical leadership and program growth for high-performing individuals.
Requirements
- Active PMHNP-BC license
- Experience in geriatric psychiatry, SNF, ALF, home-based care, or community psychiatry
- Strong diagnostic and medication management skills
- Ability to practice independently while contributing to a team-based model
Responsibilities
- Comprehensive psychiatric evaluations & medication management
- Management of behavioral symptoms in dementia and cognitive decline
- Collaboration with PCPs, palliative care, and facility partners
- Caregiver education and relationship-based care
- Documentation, care coordination, and panel management
