About The Position

Cordial Health is seeking a compassionate and skilled Psychiatric Mental Health Nurse Practitioner (PMHNP) or Physician Assistant with psychiatric experience to provide on-site behavioral health care throughout Columbia and Ulster Counties, New York. This full-time position offers meaningful clinical work, provider autonomy and extensive support from an experienced behavioral health team. You will serve adult and geriatric residents who often have limited access to specialized psychiatric services.

Requirements

  • Board-certified PMHNP-BC or Physician Assistant with significant psychiatric experience
  • Active and unrestricted New York professional license
  • Active DEA registration required
  • Strong knowledge of psychiatric assessment, diagnosis, psychopharmacology and treatment planning
  • Ability to manage a psychiatric caseload independently
  • Excellent communication, organization and clinical-reasoning skills

Nice To Haves

  • Geriatric, skilled nursing, rehabilitation, long-term care or post-acute experience preferred
  • Familiarity with electronic medical record systems preferred

Responsibilities

  • Conduct comprehensive psychiatric evaluations and follow-up visits
  • Diagnose and manage psychiatric and behavioral health conditions
  • Develop individualized, evidence-based treatment plans
  • Prescribe, monitor and adjust psychotropic medications
  • Evaluate treatment response, medication effectiveness and side effects
  • Collaborate with physicians, nursing staff, therapists, social workers and families
  • Participate in interdisciplinary treatment planning
  • Educate residents, caregivers and staff regarding diagnoses, medications and behavioral interventions
  • Provide compassionate, trauma-informed and resident-centered care
  • Maintain accurate, timely and compliant clinical documentation
