About The Position

Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Psychiatric Mental Health Nurse Practitioner (PMHNP) for Element Care PACE in Lynn, Massachusetts. This role involves providing comprehensive psychiatric evaluations, medication management, and behavioral health care for medically complex older adults enrolled in the Program of All-Inclusive Care for the Elderly (PACE). The NP will develop and manage individualized psychopharmacology treatment plans, monitor therapeutic response and medication safety, and perform psychiatric consultations and crisis evaluations in collaboration with an interdisciplinary care team. Care will be provided in PACE centers, participant homes, and skilled nursing facilities throughout Lynn and Brighton. The role requires partnering with Primary Care Physicians, Registered Nurses, Social Workers, Therapists, and other specialists to coordinate participant-centered care and maintaining accurate documentation within the electronic medical record to support the organization's mission of helping seniors remain safely in their communities.

Requirements

  • ANCC-certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) or Clinical Nurse Specialist.
  • Active Massachusetts Advanced Practice Registered Nurse license.
  • Active Massachusetts prescriptive authority and DEA registration.
  • Experience performing psychiatric evaluations, medication management, and crisis intervention.
  • Excellent communication and interdisciplinary collaboration skills.

Nice To Haves

  • Minimum two years of psychiatric prescribing experience preferred.
  • Experience working with geriatric or medically complex adult populations is preferred.

Responsibilities

  • Provide comprehensive psychiatric evaluations, medication management, and behavioral health care for medically complex older adults enrolled in the Program of All-Inclusive Care for the Elderly (PACE).
