About The Position

The essential functions of this position include, but are not limited to: completing nursing intake assessments for adolescent patients entering the Intensive Outpatient Program; establishing a therapeutic relationship with all patients and families; providing nursing care, assessment, support, and education to patients/families; maintaining a therapeutic environment through general knowledge of patients' therapy plans and reinforcement of desired behaviors; participating in delivery of educational services to patients and their families; performing discharge functions; collaborating with the psychiatric multidisciplinary team and attending regular treatment team meetings; providing pertinent clinical data to outside agencies as needed to assure compliance with their requirements. Other duties may be assigned.

Requirements

  • BLS: Basic Life Support (Within in 1 month of hire or transfer)
  • TEAM ADVANCED: Techniques for Effective Aggression Mgt (Within 6 months of hire or transfer)
  • RN: Registered Nurse
  • Associate's degree is required
  • Demonstrates effective and pleasant interpersonal relations.
  • Demonstrates ability to be flexible in changing work environment.
  • Interpersonal skills needed to communicate successfully with individuals and groups and interact with people at all levels to communicate ideas and concepts in a clear and understandable manner.
  • Ensures program meets insurance company criteria and required documentation for reimbursement.
  • Complies with MyMichigan Medical Center Midland policies and procedures.
  • Adheres to all policies and procedures related to employment, safety, program provision and operations.
  • Is familiar with the Michigan Mental Health Code as it relates to: recipient rights, confidentiality, abuse and neglect, and Duty to Warn.
  • Demonstrates competency in Infection Prevention and Control Practices and Policies.
  • Must have been involved in the delivery of mental health services for at least 1 year within the previous 5 years.
  • Exposure to stressful situations, including those involving public contact, as well as, trauma, grief and death.
  • Able to wear personal protective equipment that includes latex materials or appropriate substitute if required for your position.
  • Is able to move freely about facility with or without an assisted device and must be able to perform the functions of the job as outlined in the job description.
  • Overall vision and hearing is necessary with or without assisted device(s).
  • Frequently required to sit/stand/walk for long periods of time. May require frequent postural changes such as stooping, kneeling or crouching.
  • Some exposure to bloodborne pathogens and other potentially infectious material. Must follow MyMichigan Health bloodborne pathogen and TB testing as required.
  • Ability to handle multiple tasks, get along with others, work independently, regular and predictable attendance and ability to stay awake.
  • Overall dexterity is required including handling, reaching, grasping, fingering and feeling. May require repetition of these movements on a regular to frequent basis.
  • Must be able to occasionally (0-33% of the workday) lift or carry 11-20 lbs., frequently (34-66% of the workday) 10 lbs. and or Walk/Stand/Push/Pull of Arm/Leg controls.

Nice To Haves

  • BSN preferred.
  • Experience with adolescents preferred.

Responsibilities

  • Completion of nursing IOP assessment for all incoming patients involving biopsychosocial data collection from patient, family, and collateral contacts as needed.
  • Completes medication reconciliation.
  • Maintains patient medical records and records information legibly, accurately, using the approved format, timely, and following all procedures.
  • Communicates assessment needs to treatment team.
  • Assists in screening IOP program applicants for applicability in program, including phone calls and coordination of care needs.
  • Addresses questions about program to family members and other interested parties.
  • Participates improving the patients' physical/mental health and maintaining patient safety.
  • Responds appropriately to patient conditions utilizing critical thinking skills.
  • Coordinates medical issues for patients with the Primary Care team.
  • Reviews medications with every patient and family weekly/as needed.
  • Notes and coordinates physician orders.
  • Provides ongoing case management and assists in discharge planning.
  • Ensures all discharge forms are completed and distributed to the appropriate parties.
  • Assists patient with completing paperwork as needed.
  • Participates in treatment team meetings to review IOP patient progress.
  • Conducts educational sessions for patients as needed.
  • Provides support to the treatment team.
  • Participates in training activities as needed.
  • Educates casual staff regarding documentation and facilitates compliance.
  • Collaborates with psychiatrist regarding discharge criteria or length of stay.
  • Prepares patients for discharge from admission through their length of stay.
  • Works with therapist and outside agencies for follow up care concerns.
  • Assists program supervisor with Quality Assurance/Quality Monitoring.
  • Attends Coordination of Care meetings.
  • Prepares for and attends biweekly Team Meetings.
  • Participates in marketing program as time permits.
  • Employees need to demonstrate competency in Microsoft Windows.
