Psych Nurse Practitioner (Full Time) - Northwood, NH
About The Position
As a Psychiatric Mental Health Nurse Practitioner (PMHNP) with Impact Health PC in Northwood, NH, you will provide psychiatric care to residents within skilled nursing facilities. You will assess and manage behavioral health conditions, develop individualized treatment plans, manage psychiatric medications, and collaborate with facility staff, physicians, therapists, and interdisciplinary care teams. Requirements
- Active Psychiatric Mental Health Nurse Practitioner (PMHNP) license in New Hampshire.
- Minimum 2 years of clinical experience providing mental health care to adult populations.
- Proficiency with EHRs and clinical technology.
- Strong communication and interpersonal skills.
- Ability to work independently and collaboratively as part of an interdisciplinary care team.
Nice To Haves
- Experience in skilled nursing facilities, geriatric mental health, hospitals, residential care, or outpatient psychiatric settings preferred.
Responsibilities
- Provide person-centered psychiatric care using evidence-based practices.
- Conduct psychiatric assessments and diagnose behavioral health conditions.
- Develop individualized treatment plans and manage psychiatric medications.
- Manage an assigned caseload and provide ongoing psychiatric follow-up.
- Provide psychoeducation and medication education to patients and families.
- Collaborate with facility staff, therapists, physicians, and interdisciplinary care teams.
- Complete accurate clinical documentation within 24 hours.
- Maintain compliance with facility, state, and federal regulations.
