About The Position

At Stony Brook Medicine, our Social Workers are valuable members of our team, who provide empathic care to our patients, their families and visitors, with the highest degree of integrity and respect. Social Workers apply the principles and practices of their profession to provide integrated social casework, including direct counseling, therapy, advocacy, and other services to individuals, families and groups in a variety of settings. Social Workers plan, implement, coordinate and document treatment for individuals diagnosed with: mental illness and serious emotional disturbance; mental retardation and developmental disabilities; work-related illnesses or injuries; chemical addiction; or perpetrators or victims of neglect, abuse, or crime who need therapeutic intervention, habilitation, training, supervision and re-socialization, or confinement. Qualified candidates will demonstrate excellent communication skills, interpersonal skills, knowledge and understanding of patient care and effectively respond to changing patient needs by making decisions based on ethical principles and adhering to our high standard of excellence.

Requirements

  • Master’s degree from an accredited program.
  • New York State Licensure within one year of hire
  • Minimum 1+ year experience in an Inpatient Acute Care Psychiatric setting/Mobile team/CPEP or ED
  • Demonstrates ability to be trained in all behavioral health areas including: CPEP, Adult and Child Inpatient units, Consult & Liaison, Mobile Outreach Team, Emergency Department and Follow up worker role, for purposes of coverage as needed.
  • Understanding and ability to work with substance use disorder patient population
  • Demonstrates excellent written and oral communication skills.
  • Ability to work and coordinate with established Psychiatric Medical Treatment Team.
  • Demonstrates understanding of discharge planning and Long Length of stay barriers and systems.
  • Demonstrates understanding of Psychiatric specific community resources.
  • Demonstrates good assessment and psycho-social skills.
  • Working knowledge of Long Island and Metropolitan NY community resources.
  • Demonstrates patient first, patient advocacy and service excellence.
  • Computer literate.
  • Demonstrates understanding of community systems.
  • Demonstrates ability for project management and action planning.
  • Demonstrates team work and team leader focus.
  • Demonstrates understanding of data collection and statistics.

Nice To Haves

  • Experience in substance abuse/counseling intervention, Psych/Medical integration, and Behavioral Health with both adult and pediatric populations.
  • Bi-lingual.

Responsibilities

  • Patient/Family Assessments
  • Engage in person-centered treatment planning and ongoing counseling with individual and family members with a focus on reducing recidivism.
  • Documentation and referral processes based on clinical need and OMH regulatory requirements.
  • Clinical team case conferencing discussions and collaboration
  • Prepare and facilitate groups in all inpatient settings. Patient group facilitator and organizer
  • Person Centered and Collaborative Treatment Planning
  • Effectuate discharge planning including: insurance verification, linkage to community providers, insurance authorizations and utilization reviews, scheduling transportation and follow up phone calls as needed.
  • Work with community agencies in regards to patient post discharge needs. Outside agency contact and collaboration
  • Supervision of professional social work students
  • Active involvement in performance improvement activities including data collection and utilization of data for quality improvement
  • Provide coverage in all Behavioral Health and Emergency Department areas Float to all Behavioral Health Areas including Mobile Outreach Team
