About The Position

First Choice Health Centers is seeking a Psychiatric Nurse Practitioner (PMHNP) for our community health practice in East Hartford, CT. This position is accountable for independently performing a full range of tasks in delivery of direct patient mental health care in an outpatient setting.

Requirements

• Proven experience as a Nurse Practitioner implementing treatment plans, educating patients and administering and managing patient medication(s).
• A current license to practice as a Nurse Practitioner in the state of Connecticut.
• Appropriate Board Certification
• A commitment to serving diverse and vulnerable populations with compassion and respect.
• Excellent communication, interpersonal, and organizational skills.
• Connecticut Controlled Substance license and DEA license
• BLS Certification through the American Heart Association or American Red Cross.
• Employees of First Choice Health Centers must be vaccinated against COVID-19. Certain exemptions may apply.
• Candidates are required to pass a drug test, including marijuana testing, before beginning employment.

Responsibilities

• Diagnose patient conditions and provides appropriate treatment recommendations
• Interview patients about symptoms and complaints and determine whether a psychiatric disorder has a physiological or emotional basis Takes medical histories.
• Assess risk factors for mental illness and broader wellness evaluations
• Instructs and counsels patients regarding compliance with prescribed therapeutic regimens
• Develops and implements patient management plans, records progress notes, and assists in provision of continuity of care.
• Assumes responsibility for patient follow-up, coordination, and referrals to other health care providers as necessary.
• Maintains patient medical records.
• Provides patient counseling/education.
• Performs related duties as required.
