Requirements

Provide housing placement and retention support services to adults referred through the adult system coordinated access case conferencing process.
Work with adults to develop individualized housing plans and identify supportive services to promote housing stability.
Assist adults with completing housing applications and other required paperwork.
Build and maintain collaborative relationships with resident services staff, property managers, and management companies.
Provide ongoing support to adults, including assisting with rent payments and connecting them with necessary services.
Maintain accurate and up-to-date documentation of all participant interactions and services provided.
Attend weekly case meetings to review case files, discuss progress and challenges, and coordinate services with other team members.
Attend and participate in trainings, meetings, and other professional development opportunities as required to maintain current knowledge and skills related to housing and homelessness issues.
Adherence to El Programa Hispano Católico policies and procedures.
Perform other duties as assigned.
