Shared Health leads the planning and coordinates the integration of patient-centered clinical and preventive health services across Manitoba. The organization also delivers some province-wide health services and supports centralized administrative and business functions for Manitoba health organizations. Participating as a member of a multi-disciplinary team in a variety of mental health and addictions community and hospital settings within the context of an Indigenous health care perspective, provides culturally appropriate counselling and intermediary services to First Nations, Red River Métis, Urban Indigenous and Inuit clients. The Provincial Indigenous Health Liaison: Plans, organizes and evaluates clinical and psychosocial health care needs and services for Indigenous clients involving assessments, health teaching to clients, support, consultation, liaison, and referral to appropriate services. Collaborates with other programs across Mental Health and Addictions. Facilitates connections between Indigenous clients and service providers to enhance access to Mental Health and Addictions with a culturally appropriate approach. Provides direct care to patients through a combination of telephone, virtual video/audio conference and/or remote technology to patients in their homes, in the community including outreach, and virtually.
