About The Position

Bloom Healthcare is seeking a Provider Lead to manage a team and territory, focusing on professional development, job satisfaction, team retention, rewarding high performers, and preserving Bloom's culture. The role involves managing a team and territory, identifying growth needs, coaching team members, and rebalancing patient panels. Providers will spend 60% of their time seeing patients with a reduced patient panel to maintain clinical skills while taking on leadership. The Provider Lead will support professional development, promote a culture of excellence, identify growth areas, provide coaching and mentoring, and build a supportive team environment. This role is not intended to manage all aspects of the Bloom model; content experts will be involved for specific areas like clinical matters, population health, and HR.

Requirements

  • Minimum of 1 year of experience with Bloom Healthcare working as a Nurse Practitioner, Physician Assistant or Physician.
  • Minimum of one successful cycle as a new provider coach or student precepting.
  • Good standing with the organization with no remedial actions.
  • Must work a minimum of 80% of full-time hours.
  • Strong leadership, communication, and organizational skills.
  • Ability to work independently and as part of a team.
  • Consistent standing of 80% of note closure rate, <48h.
  • Must consistently adhere to patient visit standards.

Responsibilities

  • Oversee and manage a team of providers, including interviewing and evaluating new provider hires.
  • Manage a territory, identifying the need for growth hires, and rebalancing patient panels.
  • Manage team engagement by providing regular feedback, coaching, and recognition.
  • Communicate company updates and initiatives to the team and ensure compliance with Bloom best practices.
  • Support the coaching and onboarding of new providers to the team.
  • Mobilize resources for providers including clinical, population health, HR, and ambassador support.
  • Serve as a liaison between providers, clinical leadership, support, ambassadors, hospice, and leadership.
  • Identify opportunities to improve clinical performance and patient reception for providers.
  • Conduct annual/periodic performance reviews with each member of your team to assess and provide feedback on clinical performance, community relations, and other key performance areas.
  • Various administrative/management tasks including, but not limited to, approving PTO/leave, payroll, coverage coordination, as assigned.
