About The Position

CareBridge Health, a member of the Elevance Health family within Carelon, focuses on enabling individuals in home and community-based settings to maximize their health, independence, and quality of life through home-care and community-based services. This role is responsible for collaborating with internal and external business partners to identify and implement small to medium projects that support organizational goals, problem analysis, and resolution. The position will involve interaction with members, their families, and/or caregivers. The role is virtual, offering maximum flexibility and autonomy, with the exception of required in-person training sessions. Candidates must be within a reasonable commuting distance of the posting location(s) unless accommodation is granted.

Requirements

• BA/BS in a related field and minimum of 3 years professional project management experience, with at least 2 years spent leading and directing project tasks; or any combination of education and experience which would provide an equivalent background.

Responsibilities

• Provides performance feedback to team members and resource managers.
• Use data-driven insights to inform continuous improvement efforts and strategic decisions.
• Facilitates resolution of issues.
• Obtain/secure and archive necessary approvals.
• Manage and update project plan as appropriate.
• Work closely with cross-functional teams to ensure alignment and effective communication across all departments.
• Analyze and evaluate current operational processes, identify areas for improvement, and implement strategies to enhance efficiency, productivity, and quality.
• Develop and document standard operating procedures (SOPs) to streamline workflows.
• Other duties as assigned.
