About The Position

CareBridge Health is a proud member of the Elevance Health family of companies, within our Carelon business. CareBridge Health exists to enable individuals in home and community-based settings to maximize their health, independence, and quality of life through home-care and community-based services. Responsible for working with internal and external business partners to identify and implement small to medium projects that support achievement of simple to complex organizational goals, problem analysis, and resolution. This role will interact with our members, their families, and/or caregivers.

Requirements

  • Requires a BA/BS in a related field and minimum of 3 years professional project management experience, which at least 2 years are spent leading and directing project tasks; or any combination of education and experience, which would provide an equivalent background.

Responsibilities

  • Provides performance feedback to team members and resource managers.
  • Use data-driven insights to inform continuous improvement efforts and strategic decisions
  • Facilitates resolution of issues.
  • Obtain/secure and archive necessary approvals.
  • Manage and update project plan as appropriate
  • Work closely with cross-functional teams to ensure alignment and effective communication across all departments
  • Analyze and evaluate current operational processes, identify areas for improvement, and implement strategies to enhance efficiency, productivity, and quality.
  • Develop and document standard operating procedures (SOPs) to streamline workflows.
  • Other duties as assigned.
