About The Position

The Program Support Coordinator at University Hospital supports the expansion and delivery of Centering Programs by coordinating program activities, managing data, and reporting, and fostering partnerships with community organizations. This role assists with community health needs assessments, supports grant writing efforts, and ensures accurate tracking of program outcomes. This position plays a key role in strengthening community relationships, supporting evidence-based, patient-centered group care models.

Requirements

  • Bachelor’s Degree in Public Health or a related field required.
  • Experience with Centering Pregnancy and/or Centering Parenting programs preferred.
  • Requires four (4) years of progressively more responsible professional experience, preferably in a health care environment, which includes experience in project coordination and assisting with the administration of specific programs.
  • Excellent written and verbal communication skills required, along with a strong working knowledge of Microsoft Office.
  • Must be detail-oriented, with the ability to prioritize and problem solve.
  • Ability to work collaboratively with diverse populations and stakeholders.
  • Knowledge of community health principles and practices.
  • Additional related experience may be substituted for the degree on a year for year basis

Nice To Haves

  • CHES (Certified Health Education Specialist) certification preferred.
  • Master Degree in Public Administration, Health, or Business preferred.

Responsibilities

  • Coordinates with stakeholders to develop community health interventions based on identified needs.
  • Assists in the planning, development, and execution of Centering Programs.
  • Collaborates with healthcare providers, community partners, and other stakeholders to enhance program delivery.
  • Supports the evaluation and continuous improvement of population health initiatives through regular assessments and feedback.
  • Organizes and oversees Centering Program events, workshops, and educational sessions.
  • Assists with the implementation and administration of Centering programs.
  • Ensures compliance with regulatory guidelines and best practices.
  • Under the guidance of Management, assists with training and the support of healthcare providers and facilitators involved in Centering programs.
  • Monitors program outcomes and prepare reports for internal and external stakeholders.
  • Assists with survey and accreditation activities in collaboration with practice staff and clinicians.
