The state supported living centers serve people with intellectual and developmental disabilities who are medically fragile or who have behavioral problems. The centers provide campus-based direct services and supports at 13 centers located throughout the state. This is a career filled with purpose. The Admissions/Placement Coordinator (APC) works under the direction of the Assistant Director of Programs. Completion of work requires occasionally originating new methods and/or techniques, and decisions are varied but based on well-defined guidelines. Responsibilities and duties include, but are not limited to, planning, directing, and evaluating the admission and transition processes to ensure that activities are in compliance with ICF-IDD standards, Settlement Agreement provisions, SSLC state level policies, and Mexia State Supported Living Center policies. The APC provides information and guidance to individuals, their families, and other staff or agencies when seeking admission to a state facility in Texas, transfer from one facility to another, or to a facility out of state, and/or discharge. The APC consolidates information documented in the individual’s record and that provided by the individual, involved family and friends, the Interdisciplinary Team and/or community provider and Local Authority staff to request admission, discharge or transfer, and coordinates all activities surrounding admissions, discharge or transfer. Develops and maintains record system for individuals requesting admission, discharge, or transfer, and, monitors and approves closing cases after goals identified are accomplished. Responsibilities and duties include, but are not limited to interviewing individuals requesting services, their relatives, and other interested individuals and agencies to obtain information for admissions, discharge or transfer request. Acts as liaison between all appropriate relatives, agencies, and community resources for the purpose of admission, transfer, and discharge planning. Provides leadership to attain compliance with Section T of the Settlement Agreement.

Requirements

  • Must possess valid Texas driver’s license or obtain it no later than 90 days after hire date.
  • Applicants with an out-of-state driver’s license must provide an original certified driving record from the state of driving licensure.
  • Eligible driving record required based on HHSC Fleet Management policy.
  • Knowledge of casework principles; intellectual/developmental disabilities; community resources; the principles, methods, techniques, and practices of social work; rehabilitation methods; and Community Standards and ICF/IDD Regulations.
  • Skill in interviewing.
  • Ability to communicate effectively; to prepare written and oral reports and make recommendations accordingly; and to plan, assign, and/or supervise the work of others.
  • Skills in program development and evaluation, staff training, individual assessment, and evaluation.
  • Ability to create, develop, and implement training materials related to the admission and transition processes.
  • Ability to develop, implement, and oversee action plans in cooperation and conjunction with other departments.
  • Ability to work effectively with staff at all levels to maintain a harmonious work environment.
  • Applicants must pass a fingerprint criminal background check, pre-employment drug screen, and registry checks including the Client Abuse/Neglect Reporting System (CANRS), Nurse Aide, Medication Aide and Employee Misconduct and HHS List of Excluded Individuals/Entities (LEIE).
  • Males between the ages of 18 – 25 must be registered with the Selective Service.
  • All State Supported Living Center Employees are subject to Random Drug Testing.
  • All applicants must be at least 18 years of age to be considered for employment at a state-operated facility.

Nice To Haves

  • Bachelor’s degree in the field of human services preferred.
  • Three years of experience in working with people with intellectual disabilities in an ICF/IDD setting.

Responsibilities

  • Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime, in accordance with agency leave policy and performs other duties as assigned.
  • Coordinates activities related to individual’s transition from Mexia State Supported Living Center.
  • Coordinates admissions to Mexia State Supported Living Center. Performs systematic duties related to admissions and transitions (CARE entries, weekly reports, etc.).
  • Facilitate and lead meetings in both small and large group settings, virtually and in person.
  • Functions as facility liaison to Local Authority entities and Criminal Justice stakeholders around the state.
  • Communicates with HHSC Continuity of Care Coordinator and other A/P Coordinators concerning issues regarding admissions, discharges and community placements, on a state-wide basis.
  • Maintains active and informative contact with individuals' families and/or correspondents and documents accordingly.
  • Contributes to the individual's overall plan of care by frequent contact with residential, program, and clinical staff, and participates as an active member of the Interdisciplinary Team (IDT) in regard to admission and transition activities.
  • Act as facility lead in the development and implementation of plans and coordination of Settlement Agreement activities for Section T.
  • Actively participating and/or serving in a supporting role to meet the agency’s obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.
