The state supported living centers serve people with intellectual and developmental disabilities who are medically fragile or who have behavioral problems. The centers provide campus-based direct services and supports at 13 centers located throughout the state. This is a career filled with purpose. The Admissions/Placement Coordinator (APC) works under the direction of the Assistant Director of Programs. Completion of work requires occasionally originating new methods and/or techniques, and decisions are varied but based on well-defined guidelines. Responsibilities and duties include, but are not limited to, planning, directing, and evaluating the admission and transition processes to ensure that activities are in compliance with ICF-IDD standards, Settlement Agreement provisions, SSLC state level policies, and Mexia State Supported Living Center policies. The APC provides information and guidance to individuals, their families, and other staff or agencies when seeking admission to a state facility in Texas, transfer from one facility to another, or to a facility out of state, and/or discharge. The APC consolidates information documented in the individual’s record and that provided by the individual, involved family and friends, the Interdisciplinary Team and/or community provider and Local Authority staff to request admission, discharge or transfer, and coordinates all activities surrounding admissions, discharge or transfer. Develops and maintains record system for individuals requesting admission, discharge, or transfer, and, monitors and approves closing cases after goals identified are accomplished. Responsibilities and duties include, but are not limited to interviewing individuals requesting services, their relatives, and other interested individuals and agencies to obtain information for admissions, discharge or transfer request. Acts as liaison between all appropriate relatives, agencies, and community resources for the purpose of admission, transfer, and discharge planning. Provides leadership to attain compliance with Section T of the Settlement Agreement.
