About The Position

We are seeking a Program Specialist who will independently manage the planning, implementation, and execution of a significant program within a department. The Program Specialist is responsible for all administrative and financial functions, events, reporting, supervision and/or oversight of operational staff with program responsibilities. The Program Specialist will independently manage the planning, implementation, and execution of the care navigation program components for the Neighborhood Care Pathways (NCP). The Program Specialist is responsible for all day-to-day administrative, programmatic functions, serious adverse events reporting, budgeting and management and tracking of client program expenses, supervision and/or oversight of an assigned client panel in the NCP program. You will partner and work directly with older adults 50 years and older to create an individualized plan of care with goals to help them remain independent at home in their community. This is a hybrid position of teleworking and visiting clients in their homes in Baltimore City neighborhoods.

Requirements

  • Bachelor's Degree.
  • Two years of related experience.
  • Highly proficient with Microsoft Office programs including Outlook, Word, Excel, and PowerPoint.
  • Deep knowledge of the local community, health systems and community-based resources and supports

  • Current certification as a Community Health Worker in the State of Maryland within 6 months of hire.

Responsibilities

  • Responsible for setting goals and planning to meet program objectives.
  • Develop, maintain, and oversee timelines for all special activities, ensuring timely completion of deliverables.
  • Develop budget and manage program expenses, projections, and reporting.
  • Plan and manage outreach activities and internal events, working with vendors and internal resources as needed.
  • Create, develop and expand communications via websites, social media channels, and publication materials as needed.
  • Represent the program internally and externally.
  • Ensure timely, accurate and appropriate reporting of program activities.
  • Monitor, evaluate and implement business practices and procedures to support the day-to-day operational needs of the program.
  • Review and evaluate program needs, while maintaining cost effectiveness.
  • Manage and oversee the timely compilation of annual reports and work plans.
  • May supervise and/or oversee operational staff with program responsibilities.
  • Develop, manage, and oversee timelines for all program activities for assigned client panel, ensuring timely completion of deliverables, inclusive of outreach, screening, consent, enrollment, ongoing participant engagement and evaluation of desired outcomes for clients 50+ for the Neighborhood Care Pathways (NCP) research study.
  • Responsible for setting goals, reviewing and evaluating program objectives and performance of care navigation delivery including leading the development, implementation, and adjustment of personalized client wellness plans using best practice for complex health navigation, resource linkage and culturally responsive health education on healthy aging, health advocacy skills, chronic disease self-management, coping strategies, and available community resources to maximize client panel health and goal achievement potential.
  • Locate, coordinate, and navigate medical, behavioral health, and social services and other home and community-based services for older people who may be experiencing financial, housing and resource instability and/or living with chronic comorbid conditions.
  • Represent the program internally and externally including management and planning of outreach activities and internal events, working with community partners, vendors and internal resources and lead in the development of newsletters, social media, and community-facing materials as needed.
  • Ensure program compliance with Institutional Review Board standards and HIPAA privacy and data security requirements, maintaining accurate and timely documentation in a secure data management platform
