About The Position

The Program Nurse - LPN will coordinate the delivery of nursing and social services for adult day participants. This is a part-time position, requiring 16 to 22.5 hours per week (2 to 3 days per week). The role involves conducting new client intakes and assessments, evaluating nutritional, medical, physical, and mental functional needs of clients, and providing clear, compassionate, and effective nursing care.

Requirements

  • Graduate of accredited nursing school
  • 2-3 years of nursing experience
  • State of Connecticut Nursing License
  • CPR certification
  • Ability to travel to client homes and other community sites as necessary related to networking and intakes.
  • Availability of personal vehicle for work related travel (mileage reimbursed).
  • Ability to juggle multiple tasks and accomplish deadlines in an organized fashion.
  • Utilizes Microsoft suite, including outlook, and excel; flexibility and initiative to adapt to changing software technology.

Responsibilities

  • Coordinates delivery of nursing and social services for adult day participants.
  • Conduct new client intakes and assessments to determine the appropriateness of referrals and encourage participation at start of program and once enrolled.
  • Conduct/assess the nutritional, medical, physical and mental functional needs of the clients during the initial assessment and ongoing as necessary within scope of nursing practice.
  • Demonstrates clear, compassionate, and effective nursing, demonstrating excellent judgement.
  • Communicates effectively with clients, employees, and care partners.
  • Maintains positive working relationships both within the organization and community.
  • Adopts an empathetic, culturally aware, inclusive, and person-centered approach to all interactions.
  • Respects client privacy and dignity.
  • Utilizes a creative, supportive, and educational approach to working with clients with a variety of needs and reactions to new life circumstances.
