About The Position

Under the general direction of the Community Health Supervisor, the Program Manager oversees the daily administration of Maternal Child Health programs and initiatives in the Community Health Division, including the Family Connects Ohio (FCO) Nurse Home Visiting Program, the Complex Medical Help Program (CMH), the Cribs for Kids grant (CFK), and the Plan of Safe Care Program(POSC). Program Manager is responsible for staff training and performance, program compliance, grant management, community partnerships, and strategic planning.

Requirements

  • Successful completion of a Science in Nursing Program, Bachelor’s degree preferred
  • Five (5) years of nursing experience required
  • Valid, unrestricted, Registered Nurse Licensure pursuant to Section 4723.13 of the Ohio Revised Code
  • Experience with budgeting, grant management, data analysis and reporting.
  • Strong written and verbal communication skills.
  • Ability to direct a team of clinical and non-clinical staff.
  • Ability to work with culturally diverse populations
  • Must have competent computer skills and be able to work in Microsoft Word, Excel, Google suite (calendar, docs, sheets, Gmail, etc.) and other electronic medical documentation systems.
  • Must have proof of and maintain up to date vaccination status for healthcare providers, or be in the process of completing.
  • Valid Ohio driver’s license
  • Current CPR certification.

Responsibilities

  • Oversee daily operations of the FCO, CMH, CFK and POSC programs
  • Become fully trained in the FCO, CMH, CFK, and POSC programs and be able to perform home visits as needed
  • Assist with training and onboarding of new staff.
  • Monitor performance of clinical staff. Provide education, support and feedback as needed
  • Identify process improvement opportunities and help resolve complex problems.
  • Ensure compliance with grant requirements, Ohio Department of Health standards, and public health accreditation standards.
  • Maintain regular communication with Family Connects International Implementation team
  • Facilitate a Community Advisory Board (CAB)
  • Assist with the development and monitoring of program budgets.
  • Track performance metrics and prepare required reports for programs and grants
  • Identify emerging public health needs in the community.
  • Develop and implement evidence-based interventions.
  • Seek funding opportunities and assist with grant applications.
  • Participate in emergency response as required.
