We are building a dedicated Women's Health track within our Enhanced Care Management (ECM) program, working through community-based social service organizations that serve women. As ECM Program Manager for Women's Health, you will own outreach, engagement, and care coordination for women enrolled in or eligible for ECM. This is a field-facing, relationship-driven role. You will do direct outreach through community organizations, build partnerships, and manage a small team of community health workers. The right person has done this work before: you know how to find, engage, and retain women in care who have every reason to distrust the system.
