More info at Akidolabs.com. The Opportunity Akido's Safety Net Programs serve some of California's most vulnerable Medi-Cal members. We are building a dedicated Women's Health track within our Enhanced Care Management (ECM) program, working through community-based social service organizations that serve women. As ECM Program Manager for Women's Health, you will own outreach, engagement, and care coordination for women enrolled in or eligible for ECM. This is a field-facing, relationship-driven role. You will do direct outreach through community organizations, build partnerships, and manage a small team of community health workers. The right person has done this work before: you know how to find, engage, and retain women in care who have every reason to distrust the system.

Requirements

  • 3+ years of direct outreach and engagement experience with women in safety-net, community health, or social services settings.
  • 2+ years of experience managing front-line staff in community health, social services, or outreach settings.
  • Demonstrated ability to build trust and sustain engagement with women experiencing homelessness, SUD, SMI, domestic violence, or system involvement.
  • Experience facilitating group sessions, workshops, or community events for women.
  • Understanding of the barriers women face in accessing care: safety, childcare, transportation, distrust of systems, partner dynamics.
  • Strong organizational skills and comfort managing a caseload, tracking metrics, and maintaining documentation.
  • Valid driver's license, automobile insurance, and reliable transportation.

Nice To Haves

  • Experience in maternal/perinatal health, reproductive health, or DV/IPV services.
  • Familiarity with community-based social service organizations serving women in Los Angeles.
  • Advanced training or certification (e.g., MSW, LCSW, CHW certification, doula training, DV advocacy).
  • Bilingual English/Spanish strongly preferred.

Responsibilities

  • Design and execute outreach strategies to engage and enroll women eligible for ECM through community-based social service organizations, shelters, and other community settings.
  • Build trust with women who have experienced trauma, system involvement, or housing instability. Meet people where they are.
  • Plan and facilitate group sessions, wellness events, support groups, and engagement activities tailored to women.
  • Develop relationships with community organizations serving women, family resource centers, shelters, and other referral partners.
  • Collaborate with clinical teams to ensure warm handoffs from outreach to enrolled care.
  • Collaborate closely with medical provider to ensure patients' health needs are met efficiently.
  • Oversee care plans for enrolled women, ensuring they address medical, behavioral health, housing, legal, childcare, benefits, and safety needs.
  • Ensure documentation meets Akido's quality and compliance standards.
  • Facilitate daily huddles, weekly case reviews, and care planning meetings.
  • Track enrollment, engagement, and outcomes metrics. Use data to adjust strategies.
  • Supervise a small team of community health workers focused on women's health outreach and enrollment.
  • Coach staff in trauma-informed engagement, motivational interviewing, and harm reduction.
  • Support hiring and onboarding as the program grows. Lead by example in the field.
