About The Position

The Program Manager, RN at Memorial Hermann Southeast/Pearland will participate in multi-disciplinary rounds, facilitate patient satisfaction escalations, review quality metrics, conduct quality audits, monitor dashboards, play an active role in case management discharge planning, and participate in continuous quality improvement with external referral sources, social workers, discharge planners, case managers, healthcare professionals, families, and patients. The role involves leading and coordinating onboarding for Physicians and Advanced Practice Providers (APPs), overseeing Business Coordinator functions, managing program billing processes, and partnering with the Physician Lead to develop and maintain Physician and APP schedules. The Program Manager will collaborate with the healthcare team to achieve positive outcomes, oversee clinical workflow in conjunction with the Medical Director, serve as a super user for EMR, and participate in systemic evaluation of programs and process improvement. This role also includes leading educational programs, developing relationships with referral sources and internal staff, connecting hospitalists' plans with other providers, implementing and evaluating changes in clinical and operational processes, and ensuring safe patient care. The position may involve coverage of additional campuses and promotes professional growth and development.

Requirements

  • Graduate of an accredited School of Professional Nursing, Bachelor's of Nursing, required
  • Certified in Basic Life Support
  • Current State of Texas license or temporary/compact license to practice professional nursing
  • Minimum of five (5) years nursing experience in an acute care setting
  • Two (2) years of previous supervisory experience required
  • Understanding of quality improvement, performance improvement, change management and patient safety knowledge preferred
  • Clinical knowledge of assigned clinical conditions and physiological systems to discern the impact of specific indicators on outcomes and to identify potential gaps in clinical care processes
  • Ability to interact with all members of the healthcare team and administrative staff in a professional and courteous manner
  • Interpersonal skills necessary in order to interact effectively with external referral sources, including social workers, discharge planners, case managers, other healthcare professionals, families and patients
  • Demonstrate strong skills and a track record for sustained measurement of program outcomes, ability to work independently and with and through others to achieved desired results
  • Ability to build relationships and gain alignment for clinical support from superiors and peers
  • Ability to multi-task, prioritize work, pay close attention to details and meet deadlines
  • Excellent organizational and communication skills
  • Ability to effectively communicate complex ideas (both conceptual and concrete) by oral and written means
  • Ability to work independently with little or no supervision and demonstrate excellent problem solving and critical thinking skills when making decisions

Nice To Haves

  • Additional previous case management, quality and/or clinical documentation knowledge is preferred
  • Working knowledge of Cerner Electronic Health Record preferred
  • Proficient in Microsoft Office, including Excel, Word, PowerPoint and Visio

  • Ensure safe care to patients, staff and visitors
