Spectrum Health Care is currently seeking a Registered Nurse (RN) to join our team as Program Manager, Integrated Care to support our teams in Mississauga. This is a 6 month contract with possibility of extension or permanency. This individual will be responsible for leading a diverse, multidisciplinary team that will deliver high-quality, integrated care in the community, driving both operational and clinical excellence. Requirements

  • Current CNO certificate as Registered Nurse, degree preferred
  • Minimum of 5 years of recent experience with progressive responsibilities as a Nursing Manager, preferably in a community setting
  • Solid knowledge of the principles, practices, and methods related to leading multidisciplinary teams
  • Experience with program development, implementation and evaluation
  • Valid G driver's license and reliable vehicle

  • Experience managing in a union environment is an asset

Responsibilities

  • Cultivates and manages high-performing teams comprised of Nurses, Personal Support Workers (PSWs), and Allied Health professionals – fostering a collaborative, innovative, and learning-focused environment that aligns with Spectrum's Mission, Vision, and Values.
  • Ensures seamless program operations by leveraging data-driven insights to optimise service delivery, enhance quality, and achieve key outcomes.
  • Collaborates with external health system stakeholders to support the design and implementation of innovative programs, ensuring seamless care transitions across the broader health system.
  • Leads program development and designs clinical workflows to support innovative models of care whilst managing key internal and external stakeholder relationships.
  • Recruits, onboards, and mentors team members, ensuring they possess the necessary attributes, training, and skills to meet high care quality standards.
  • Addresses quality, safety, and risk issues, monitoring key performance indicators and initiating improvements as required.
  • Contributes to improved client outcomes through evidence-informed practices, resource efficiency, and timely resolution of client concerns.
