About The Position

Serves as Office of Aging and Adult Services (OAAS) Program Manager 1B overseeing Program Monitors or My Choice Louisiana Transition Coordinators domiciled in various LDH Regions. Travel is required. My Choice Louisiana (MCL) is a state program within the Louisiana Department of Health (LDH) and operated jointly by the Office of Aging and Adult Services and the Office of Behavioral Health. MCL grew out of an agreement between the LDH and the United States Department of Justice (DOJ). In this agreement. LDH committed to increasing services and supports giving individuals in Medicaid Long Term Care institutions (LA Nursing Facilities) with Serious Mental Illness (SMI) and physical limitations due to disability increased opportunities for successful transitions into communities of their choosing. MCL staff work with individuals identified as meeting criteria for inclusion in the DOJ specified target population (diagnosis of serious mental illness, 18 years of age and older, and Medicaid eligibility) and assist with desired transition into the community. The assistance provided by MCL staff includes but is not limited to assessment, identifying needed supports and services and securing such, and intensive case management both pre- and post-transition. The requirements of the agreement require LDH to have the necessary staff to carry out such tasks and the appropriate level of management and supervision to ensure tasks are delivered within a quality and consumer-driven framework. Thus, it is critical that OAAS develops the staffing necessary to support this. Requirements

Seven years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR Six years of full-time work experience in any field plus four years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR A bachelor’s degree plus four years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR An advanced degree or a Juris Doctorate plus three years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services.
Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full-time work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field.

Nice To Haves

Knowledge of long-term care service delivery to the elderly and people with disabilities, such as serious mental illness.
Experience with one or more of the following is required: transition planning from nursing facilities, support coordination/case management, social services coordination, Medicaid or program office function on the state or regional level.
Preference will be given to applicants who have experience in working with older adults, knowledge of and experience working with elderly and disabled stakeholder community; and experience with mental health services delivery and Medicaid eligibility and benefits

Responsibilities

Implementation of program's design, policy and procedures for the OAAS Nursing Facility Transition program entitled My Choice Louisiana (MCL).
On-going supervision of field staff, TCs, including but not limited to weekly monitoring of activity logs of field staff to ensure yearly target benchmarks are achieved; conducting at a minimum 2 annual trainings (as budget allows) with field staff to ensure program updates are received, and conducting field visits at random for quality monitoring and progress updates
Data mining/compiling of data from field staff’s data collection/entry into the SharePoint platform to monitor progress of transition cases and provide data required to complete required reporting to the DOJ
Identify problems and issues and take appropriate action to address any issues that may prevent annual benchmarks from being achieved. (i.e. barriers to transitioning program participants)
Review all check requests received from field staff for compliance and adherence to program guidelines. Prepare for submittal all check requests received and approved for appropriate signatures and submittal to LDH Fiscal for payment
Monitor transition status against individual data indicators and transition targets.
Maintain, identify trends and patterns on a local and statewide level of transitions including demographics, age, population, nursing facility, etc. as variables.
Prepare and present when needed at the Core Advisory stakeholders group meeting any information relevant to the group's interest.
Conduct annual and midpoint reviews on all supervised staff as required by civil service and LDH.
When necessary, conduct interviews and hire staff needed to fill any vacant positions.
Maintain Civil Service compliance with required trainings annually personally and for supervised staff.
Respond to emergency activations per LDH All Hazards policy.
