About The Position

Clarity Care is looking for a Program Lead to join their team in Green Bay, WI. This is a full-time management position with a starting pay of up to $21/hr based on experience. As a healthcare organization, Clarity Care offers careers that make a difference to real people who need help. The Program Lead will be responsible for leading group home operations, building a team environment, and providing hands-on caregiving. The role also involves community involvement and a desire to make a difference in human services.

Requirements

Must be at least 21 years of age.
Must meet agency driving standards.
Must pass a periodic WI Caregiver Background Check including Department of Justice Criminal Record Checks, and/or that of previous states of residence.

Prefer an Associate degree or higher in a health care related field; or a Bachelor’s degree in a field other than health care, and one year experience working in a health care related field having direct contact working with and teaching persons with developmental disabilities; or possess a valid nursing home administrator’s license. (strongly preferred)
Prior supervisory experience strongly preferred.

Responsibilities

Hands on Caregiving: Provides direct care support to members with disabilities by assisting with activities of daily living; bathing, rooming, toileting, and hygiene as outlined by the individual service plan. Be comfortable using mobility devices and practice safe transfer and ambulation techniques like in assisted living facilities.
Leads group home meetings and communicate changes with the home, consumers, staff, and organization to ensure everyone is informed and updated.
Collaborates with HR in hiring, retention, discipline, and termination processes to ensure qualified employees and adequate staffing.
Completes all applicable documentation related to tasks assigned.
Welcoming Home: Maintains a safe, sanitary and welcoming group home by performing housekeeping (vacuuming, emptying trash, cleaning bathroom, dusting bed making and changing linens); and laundry.
Prepares and Serves Meals by following general nutritional guidelines, menus, individual dietary restrictions, needs and preferences to ensure consumer health.
Community Activities: Assists, escorts, and provides physical and other types of assistance needed by members during social and recreational activities.
