About The Position

Clarity Care is looking for a Program Lead to join their team in Green Bay, WI. This is a full-time management position with a starting pay of up to $21/hr based on experience. The Program Lead will be responsible for leading group home operations, building a team environment, and providing hands-on caregiving. The role also involves community involvement and a desire to make a difference in human services.

Requirements

  • Must be at least 21 years of age.
  • Must meet agency driving standards.
  • Must pass a periodic WI Caregiver Background Check including Department of Justice Criminal Record Checks, and/or that of previous states of residence.

  • Prefer an Associate degree or higher in a health care related field; or a Bachelor’s degree in a field other than health care, and one year experience working in a health care related field having direct contact working with and teaching persons with developmental disabilities; or possess a valid nursing home administrator’s license. (strongly preferred)
  • Prior supervisory experience strongly preferred.

Responsibilities

  • Hands on Caregiving: Provides direct care support to members with disabilities by assisting with activities of daily living; bathing, rooming, toileting, and hygiene as outlined by the individual service plan. Be comfortable using mobility devices and practice safe transfer and ambulation techniques like in assisted living facilities.
  • Leads group home meetings and communicate changes with the home, consumers, staff, and organization to ensure everyone is informed and updated.
  • Collaborates with HR in hiring, retention, discipline, and termination processes to ensure qualified employees and adequate staffing.
  • Completes all applicable documentation related to tasks assigned.
  • Welcoming Home: Maintains a safe, sanitary and welcoming group home by performing housekeeping (vacuuming, emptying trash, cleaning bathroom, dusting bed making and changing linens); and laundry.
  • Prepares and Serves Meals by following general nutritional guidelines, menus, individual dietary restrictions, needs and preferences to ensure consumer health.
  • Community Activities: Assists, escorts, and provides physical and other types of assistance needed by members during social and recreational activities.
