Requirements

  • Three to five years of management experience working in a social service or related setting.
  • Excellent interpersonal skills
  • Ability to interact professionally with staff, consumers, and other stakeholders
  • Ability to assess priorities and manage a variety of activities in a time-sensitive environment and meet deadlines with attention to detail and quality.

Nice To Haves

  • Experience with Peer/Recovery services preferred.
  • Qualified Health Professional listed as a Licensed social worker, alcoholism counselor, mental health counselor or rehabilitation counselor preferred.

Responsibilities

  • Coordinate the day-to-day activities associated with managing the Short Term Crisis Respite program.
  • Supervise assigned staff in the teaching of individuals to look at crisis differently and utilize wellness tools to prevent future crises or to decrease the intensity of personal crisis so individuals can transition back to their permanent residential setting.
  • Ensure a friendly, safe and supportive home-like environment.
  • Maintain collaboration and coordination with clinical treatment providers and care coordinators to determine continuation of necessary care and make recommendations for modifications to individual’s Plan of Care.
  • Establish working relationships with various Managed Care Companies and obtain authorization for services as needed.
  • Supervises assigned staff in the day-to-day implementation of high quality program services; interprets and implements the Health and Recovery Plan (HARP) Adult Behavioral Health Home and Community Based Services (HCBS) Short Term Crisis Respite regulations including ensuring an HCBS assessment is completed and length of stay limitations are adhered to.
  • Provides oversight of the Plan of Care.
  • Ensures that staff receives training and support required to meet performance expectations.
