The Program Coordinator plays a vital role in supporting patients and families as they navigate healthcare, social services, and community resources. This position focuses on building trusted relationships, helping individuals overcome barriers to care, and connecting them with the services and supports they need to improve their health and well-being. The Program Coordinator works closely with patients, care teams, community partners, and local programs to ensure coordinated, compassionate support. In addition, this role helps identify gaps or challenges in access to care and shares insights with leadership to support continuous improvement and better outcomes for the communities we serve. The ECM Coordinator will be instrumental in structuring the Enhanced Care Management program, in conjunction with the Clinic Operations Managers/COO for Shasta Cascade Health Centers. This position will be working in collaboration with a Community Health Worker to contact and structure check-ins with clients. This role will be front facing with patients and families to determine if patient(s) qualify for the ECM program and then support in educating and accessing appropriate programs for their needs. All clients will be provided a structured plan of care with goals that align with the ECM program. This position requires a majority of patient-facing time in the community, as well as time spent collaborating with the various programs in the community that may serve as resources for patients, such as community resources centers, food banks, mental health services, public health services and more.
