About The Position

Under the direction of the Director of
Research (Program Director, TN-HHN project), the Program Coordinator provides
essential operational and coordination support for projects across Tennessee
designed to improve cardiovascular disease (CVD) care outcomes. This position
supports the Centers for Disease Control and Prevention Cardiovascular
Innovation Grant and works closely with the Practice Transformation Lead,
Learning Collaborative Lead, Health Equity Liaison, and Program Manager to
ensure smooth implementation of project activities. This position also supports
engagement of patients and community stakeholders in Tennessee Heart Health
Network (TN-HHN) Learning Collaboratives (LCs). Additional responsibilities
include tracking deliverables, preparing progress reports, supporting event
logistics, and facilitating collaboration between community and clinical
partners to achieve project objectives.

Responsibilities
