About The Position

The Program Coordinator Team Lead – Case Management reports to the Director of Case Management. Under general supervision, the Program Coordinator Team Lead coordinates the daily operations of the Program Coordinator Team and supports the RN Case Managers and Social Workers in their work with the interdisciplinary healthcare team across the continuum. The Program Coordinator Team Lead coordinates payer communication, transition plan implementation, delivery of patient documentation and other duties in support of case management departmental functions.

Requirements

• Bachelors Degree and 2 years work experience; or High School Diploma (or equivalent) and 5 years experience required.

Responsibilities

• Coordinates the daily operations of the Program Coordinator Team.
• Supports RN Case Managers and Social Workers in their work with the interdisciplinary healthcare team across the continuum.
• Coordinates payer communication.
• Coordinates transition plan implementation.
• Coordinates delivery of patient documentation.
• Performs other duties in support of case management departmental functions.
