About The Position

The Department of Developmental Services (DDS) is committed to providing individualized, comprehensive supports and services to children and youth participating in the Department of Elementary and Secondary Education (DESE)/DDS Residential Prevention Program. Under the direction and oversight of the Family Supports Director, the Program Coordinator II will provide regional coordination, case management, and program expertise to support children, youth, and their families in maintaining successful placements in the home and community. The DESE/DDS Program Coordinator II plays a critical role in supporting the continued growth and implementation of this statewide initiative. The program is designed to provide children and youth with individualized supports that complement their educational programming and Individualized Education Programs (IEPs), while strengthening the capacity of families to meet their child’s needs at home. Through coordinated, wraparound services and active case management, the Program Coordinator II will help support the Department’s efforts to prevent or defer the need for more restrictive and costly out-of-home placements.

Requirements

  • At least (A) three years of full time, or equivalent part-time, professional, administrative or managerial experience in business administration, business management or public administration the major duties of which involved program management, program administration, program coordination, program planning and/or program analysis, or (B) any equivalent combination of the required experience and the substitutions below.
  • I. A Bachelor's degree with a major in business administration, business management or public administration may be substituted for a maximum of two years of the-required experience.
  • II. A Graduate degree with a major in business administration, business management or public administration may be substituted for the required experience.
  • III. A Bachelor's or higher degree with a major other than in business administration, business management or public administration may be substituted for a maximum of one year of the required experience.
  • Education toward such a degree will be prorated on the basis of the proportion of the requirements actually completed.

  • Criminal background check (CORI) by regulations set forth by the Executive Office of Health and Human Services.
  • National Background Checks, Chapter 234 of the Acts of 2014, conduct fingerprint-based check.

Responsibilities

  • Coordinate the evaluation, assessment, service planning, and development of individualized supports for new DESE/DDS program participants.
  • Provide ongoing case management and coordination of services to ensure that children, youth, and families receive appropriate and timely supports.
  • Collaborate with families, DDS staff, schools, educational personnel, and approved DESE/DDS service providers to develop and maintain effective wraparound supports in the family home and community.
  • Work closely with approved DESE/DDS providers throughout the assigned region to coordinate services, address barriers, and promote continuity of care.
  • Support the Department’s efforts to enroll eligible participants in the DESE/DDS self-direction program and provide ongoing assistance with self-direction processes and portal-related issues.
  • Monitor participant services, budgets, and supports to ensure alignment with individualized plans and program requirements.
  • Provide problem-solving and technical assistance to families, providers, and other stakeholders as needed.
  • Maintain regular communication and follow-up with participants and their support networks to assess service effectiveness and address emerging needs.
  • Participate in ongoing monitoring, review, and adjustment of services to ensure that supports remain responsive to the child/youth and family’s needs.
  • Develop and maintain a strong working knowledge of DESE/DDS program requirements, self-direction, budgeting and fiscal oversight, service coordination, and available community-based resources.
  • Contribute to the continued development and successful implementation of the DESE/DDS Residential Prevention Program across the region.
  • Assist with identification of community resources that are relevant to children and families and disseminate to Area Office and provider agencies.
  • Participate in Regional and statewide outreach efforts to DDS staff, community organizations, families, schools, other state agencies.
  • Support the Regional Family Support Director and Community Systems team in projects such as RFR reviews, transition resource fairs, community outreach presentations.
