About The Position

Passavant Memorial Homes is seeking a full-time Home and Community Services Program Coordinator. This role supports the Program Director in overseeing the daily operations of the Home and Community Services program. The Program Coordinator ensures the coordination of services for assigned individuals in accordance with their Individual Plan. The position involves covering and traveling throughout Indiana County, with the home office located at the Corporate Office in Harmarville. Requirements

Master’s degree or higher from an accredited college or university and one (1) year experience working directly with individuals with developmental disabilities
OR Bachelor’s degree from an accredited college or university and two (2) years’ experience working directly with individuals with developmental disabilities
OR Associate’s degree, or 60 credit hours, from an accredited college or university and four (4) years’ experience working directly with individuals with developmental disabilities.
OR Five (5) years’ experience working directly with individuals with developmental disabilities and strong demonstration of managerial and leadership skills.
Valid PA Driver’s license.

Responsibilities

Ensure that Individual Plans (IPs) are implemented as written.
Report any change related to the individual's needs to the Supports Coordinator.
Recommend revisions to a service or outcome in the IP as needed.
Ensure all Home and Community Intermediatory Support Organization (ISO) Support Service Workers are trained on the individuals IPs.
Attend IP/Interdisciplinary Team meetings as needed.
Interact with other departments to meet individuals needs.
Ensure all pertinent information is entered in Consumer Management accurately.
Input information for HCS ISO Support Service Worker pay.
Ensure compliance with all applicable regulations and PMH policies and procedures.
Support in the engagement of potential HCS ISO Support Service Workers.
Complete ISO Support Service Worker start-up requirements.
Provide emergency coverage as needed and directed.
Participate in the on-call rotation for HCS.
Communicate effectively with all levels of personnel, and present a positive and professional attitude with outside agencies, community representatives, families/advocates, etc.
All other duties as assigned by the Home and Community Services Director.
