This is a hybrid role that requires being in-person in Atlanta, with various events from Augusta to Athens. Must be able to work some evenings/weekends.
Requirements
- Bachelors degree in nursing, social work, or other healthcare-related degree
- Minimum of five years of experience in healthcare.
- Georgia LCSW or RN license based on degree
Nice To Haves
- Experience with project and/or program management.
- Certification as a Wellness Coach or Health Education Specialist is highly preferred
Responsibilities
- Responsible for developing, implementing and managing The Women's Heart Support Network in coordination with the Women's Heart Physician Team.
- Serves as point of contact for women with heart disease, inclusive of identifying and engaging women in the Piedmont Healthcare System as well as in the community.
- Promotes quality and engagement across the continuum of care to ensure high patient satisfaction.
