Job Summary The Heart Program Coordinator will facilitate the chest pain program and other Heart & Vascular related registry/accreditation requirements in partnership with the respective Medical Director and hospital administration. The Heart Program Coordinator establishes an overall direction for related services, educational, administrative, and promotes evidence-based care for all chest pain and acute coronary syndrome patients (ACS) and other Heart & Vascular patients from admission through discharge.

Requirements

  • Bachelor’s Degree in Nursing required; Master’s preferred.
  • Current state of Texas RN license required
  • Current BLS and ACLS certification required
  • At least three (3) years of clinical experience
  • Two (2) years of experience with clinical program management or cardiac cath lab experience preferred
  • Excellent organizational and communication skills
  • Intermediate or expert level computer skills in Word, Excel, Powerpoint, Outlook

  • Certification in field of expertise preferred
  • Maintains membership in a society focused on ACS diagnosis and treatment, preferred.

Responsibilities

  • Responsible for coordinating resources and activities necessary to ensure that the Chest Pain and other Heart & Vascular programs meet all regulatory and/or accreditation requirements.
  • May be asked to review policies and procedures in relation to the care of the chest pain patient and other Heart & Vascular patients.
  • Assists in the identification of patient and staff education needs and works cooperatively with other hospital departments to coordinate education opportunities for CP/ACS patient care.
  • Collaborates with the Community Liaison for public education and prevention programs in the community.
  • Collaborates with EMS and provides feedback and follow up on patients in a timely manner.
  • Participates in the Cardiology Committee of the Regional Advisory Council. (RAC).
  • Promotes a good working relationship among medical, nursing, clerical, and pre-hospital personnel.
  • Responsible for chart review/ data abstraction or review for CP/ACS and other Heart & Vascular patients per AHA GWTG, RAC, and accrediting body requirements.
  • Maintains an updated database for performance improvement activities.
  • Maintains a Performance Improvement program to obtain objective data for overall analysis of the Chest Pain and other Heart & Vascular programs in collaboration with MHHS Registry Reporting.
  • Selects appropriate processes for review and initiates change processes as needed to promote adherence to current evidence-based standards of care for the Chest Pain/ACS and other Heart & Vascular patients.
  • Leads and/or facilitates the Chest Pain Committee meetings monthly and other Heart & Vascular PI committees, as necessary; maintains accurate records of the Committee’s work.
  • Shares with appropriate personnel and medical staff the current updated standards for CP/ACS care per the American Heart Association.
  • Meets as needed with the medical staff and administrative leadership of the Chest Pain and other Heart & Vascular Programs.
  • Participates in EMS Committee, EC Section, Cardiology Section, and other standing meetings, as requested.
  • Prepares application documents for initial or renewed Chest Pain and other Heart & Vascular accreditations and submits interim documents as required by the accrediting body.
  • Demonstrates a minimum of 8 contact hours every year in ACS or chest pain-related education.
  • Maintains membership in a society focused on ACS diagnosis and treatment, preferred.
  • Other duties as assigned to meet the needs of the program and patient population
  • Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.
  • Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.
